Merakey is seeking aFull-Time Dual Diagnosis (Mental Health and IDD) Registered Nursebased out ofZanesville, OH serving counties across Mid-East Ohio.The Dual Diagnosis Treatment Team (DDTT) program offers support to children and adults who have both a serious mental illness and a developmental disability. Participants receive counseling, physical healthcare, support finding housing, assistance during crises, and help dealing with other issues in their daily lives.
The DDTT Nurse acts as a team player and shares in the responsibility of carrying out rehabilitation and support functions and assisting in treatment, substance abuse services, education, support and consultation to families and crisis intervention services.
- Assist in the coordination and monitoring of medications (both medical and psychiatric)
- Coordinates medical care with Primary care physicians, specialists, dental and vision providers as needed
- Performs periodic health assessments to monitor physical health wellness, medication issues/needs and current fitness and nutritional/dietary needs
- Works to develop goals related to physical health wellness, medication management, fitness and nutrition with individuals served and their supports
- Participates in on-call schedule on a rotating basis
- Tracks and manages all required metabolic screening
- Provide medication education to individuals, family and supports

Requirements:Community-Based Nurse - Dual Diagnosis (IDD/MH)
- Applicant must possess a degree from an accredited school of nursing and a current unrestricted license for the state of Ohio as a Licensed Register Nurse with a minimum of 2 years of experience in mental health. Applicants must demonstrate:
- Knowledge and understanding of recovery principles and recovery driven service system
- Understanding of adults with serious mental illness and/or intellectual developmental disabilities
- Applicant must also possess a valid OH Driver's License and the ability to pass appropriate state and federal clearances.
