Overview
The Patient Liaison Coordinator is an integral part of the care coordination team and is responsible for collaborating with patients, families, caregivers, physicians and other members of the multidisciplinary team to coordinate and help manage patient needs. The intent of Patient Liaison Coordinator activities is to ensure that patients are able to access all appropriate services in a timely and cost effective manner. The Patient Liaison Coordinator will work to overcome obstacles and guide members through the multidisciplinary systems. Our purpose is to help the parents and patients to follow through with the request of their provider by providing an extended helping hand. This role requires someone who has empathy, compassion and an understanding of the cultural and socioeconomic backgrounds in the communities and is knowledgeable about community resources, including financial, educational, social, and emotional support services available to patients.
The patient Liaison Coordinator will work closely with providers from both Pediatrics and Adult Internal Medicine. This position will, in some cases, act as the liaison/patient advocate from the practice into the community based on the need.
Responsibilities
- Create trustful relationships between patient, caregivers, providers and outside systems to aid the patient in receiving necessary help or guidance.
- Work closely with care team to understand barriers both culturally and socioeconomically.
- Ability to work from written or verbal based referrals.
- Must be able to work with frequent interruptions and with emergencies as needs arise.
- Must gain basic knowledge of insurance requirements, restrictions and allowances.
- Meet with patients/caregivers as needed. Monitor on-going needs to ensure efficient and effective access to services.
- Remain composed under stress and handle self in a professional manner at all times.
- Must display the ability to work independently with minimum direction.
- Must learn and use the Epic system accordingly.
- Ability to coordinate and implement multiple responsibilities, projects and priorities.
- Must keep accurate and detailed tracking of all patient activities.
- Must document in patients chart in the Epic system and communicate with provider on each case.
- Must have the ability and desire to follow through and close each referral.
- Attend meetings as requested.
- Process all referral requests according to policy and procedure.
- Obtain authorization for specialist referrals and radiology when necessary.
Qualifications
- High school diploma or GED
- Communication Skills (written, verbal, presentation and patient engagement)
- Interpersonal and Relationship Building Skills (with patients and medical staff)
