Participates in interdisciplinary care conferences to assist in assessing the spiritual and emotional needs of patients and caregivers including psychosocial issues, adjustment to illness or disability, status of religious/spiritual coping, supporting others in the process of identifying and applying their own values in decision making.
Demonstrates sensitivity to the needs, customs, or feelings of patient and caregivers.
Makes home visits to attend to patients at end of life.
Provides support and comfort to the dying patient and caregivers, as well as prayer when requested by patient or caregiver.
Provides consultation services to develop individual plans of care addressing patients' and caregivers spiritual and emotional needs.
Provides appropriate input regarding spiritual and emotional issues to case manager, caregivers and physicians.
Educates field and office staff in how to assess and address spiritual and emotional needs of patients and caregivers.
Develops and presents orientation and continuing education programs regarding spiritual care, advance directives, bereavement, etc. for employees.
A Pay Per Visit Rates is the reasonable estimate that MercyOne in good faith believes it might pay for this particular job based on the circumstances at the time of posting. MercyOne may ultimately pay more or less than the posted range as permitted by law. Qualifications
Bachelor's Degree and a Master's Degree in Theological Studies or Master's Degree of Divinity from an accredited school, or equivalent.Completed four units (1,600 hours) of appropriately accredited Clinical Pastoral Education.
Certification by a professional certifying agency or equivalent, or ecclesiastical endorsement.
One year Chaplain experience.
Have competencies for chaplaincy, including exhibiting a high level of interpersonal, facilitative and communication skills and ability to do theological reflection.
Demonstrated ability to work with patients at end-of-life and with the elderly. Exhibits a high degree of independence and reliability.
Strong written and verbal skills.
Ability to work closely with a wide range of professionals or a multi-disciplinary team.
Interpersonal skills that facilitate inter- and intra-team/departmental communication.
Excellent organizational and time management skills.
Excellent attention to detail.
Certification by NACC or APC or eligibility for certification.
