We are currently seeking a full time day Discharge Planner to Facilitate admissions and discharges for our Addictions and Recovery Center located in Richmond, IN. The hours for this position are 8a-5p and will require some travel for coordination of services.
Key Responsibilities:
- Facilitate admissions to the unit by: obtaining pertinent psychiatric and medical information from the referral source, making contact with the physician and unit and clarifying the availability of space and the time and legal status of the admission.
- Completes initial evaluation of patient through chart review, patient/family contact, team meetings, contacts and completes all paperwork related to transfers to other psychiatric/drug and alcohol inpatient/outpatient facilities.
- Evaluate the needs of the client. This will include housing, obtaining medication, supplies, appropriate referral for treatment and other programs to assist client in getting basic needs met.
- Participates in all treatment team meetings (rounds, treatment plan conferences) related to assessment, planning and evaluation of patient care.
- Responsibility for completing progress notes, discharge summaries and closing of patient records.
- Complete documents for the court and be able to provide testimony in commitment hearings.
- Contacts referral sources with information of patients progress or disposition.
- Prepares and reviews with patient and family the continuing care plan at discharge.
- Make referral to appropriate treatment programs and provide clinical information pertinent to the referral. Complete the appropriate forms and other paperwork necessary for the referral.
- Facilitate psycho-education groups.
- Provide case management services.
- Receives and acts on feedback from the provider, nursing staff, and therapist related to discharge planning.
We are seeking candidates with the following qualities:
- Bachelors in Social Work, Psychology, or related field.
- LCSW, LMHC preferred.
- One (1) year behavioral healthcare experience also preferred.
- Possess a valid drivers license, insurance, a safe driving record, and reliable vehicle.
