Position Overview:
The Intake Specialist is responsible for obtaining client demographic information, screening clients for level of care referral appropriateness, and conducting a thorough admission process while adhering to referring entity guidelines and requirements.
Principal Duties and Responsibilities (Essential Duties):
1. Complete accurate and comprehensive intakes, which include a social, medical, legal, and substance use history of the client, and enhance the quality of the psychosocial assessment conducted on all clients;
2. Provide a “welcoming” experience and exceptional customer service to all who visit Meridian;
3. Ensure the completion of all designated forms and screening processes during intake and timely distribution;
4. Ensure that documentation specific to referring agency and/or funding status is identified, completed and distributed to the appropriate staff;
5.Thoroughly and accurately enter selected information into “CPI Trac” and other electronic medical record systems, as required;
6. Conduct thorough and accurate inventories of property of newly admitted clients;
7. Maintain a supply of all materials necessary to conduct intakes;
8. Maintain all reference information pertinent to client tracking and data assessment;
9. Collect and record client co-pays with accuracy;
10.Other duties, as assigned.
Qualifications:
- Bachelor degree/Associate’s degree in appropriate academic discipline preferred or equivalent experience in behavioral health setting.
- At least one-year practical experience in therapeutic setting with emphasis on intake and screening in a rehabilitation setting, unless otherwise waived.

Physical Requirements:
- Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA and other federal, state and local standards, including meeting qualitative and/or quantitative productivity standards.
- Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards
- Must be able to lift and carry up to 50 lbs
- Must be able to talk, listen and speak clearly
