Position Purpose: Perform duties to conduct and manage the day to day operations of the case management functions
- Review analyses of activities, costs, operations and forecast data to determine progress toward stated goals and statistical/financial purposes for case management
- Ensure compliance with federal and state regulations, contractual agreements and National Committee for Quality Assurance (NCQA) standards
- Develop, implement and maintain compliance with policies and procedures regarding medical and social case management
- Develop, implement, and maintain case management programs to facilitate the use of appropriate medical and community resources and decrease health plan financial exposure
- Ensure quality of case management being delivered to members through auditing, retraining and providing ongoing opportunities to department
- Provide team with educational resources, evaluate effectiveness and recommend changes

Education/Experience: Bachelor’s degree in Nursing, Social Work, Behavioral Health, related field or equivalent experience. 3 years of case management experience. Previous experience as a lead in a functional area, managing cross functional teams on large scale projects or supervisory experience including hiring, training, assigning work and managing the performance of staff. Familiarity with Medicaid and Medicare managed care practices and policies preferred. Experience with long term care programs preferred.
**This position is remote however; candidates must reside in Illinois as it required to travel within the state of Illinois.**
License/Certification: Current state’s RN license and/or Case Management Certification (CCM), LCSW, or LCPC preferred.
