POSITION: Field Nurse
POSITION SUPERVISES: Licensed Practical Nurse and Home Health Aides
QUALIFICATIONS:
- Must be graduate of an accredited nursing program
- Must be a current Illinois license as a Registered Nurse.
- Must have at least one (1) year of clinical experience, preferably in rehabilitation or medical surgical nursing (home health experience preferred).
- Have current CPR certification
- Possesses interpersonal, communication and organizational skills necessary to complete job responsibilities
- Has an insured automobile and valid driver’s license or access to adequate transportation
- Has the ability to make visits and provide skilled nursing assessments and skilled care in the home.
- Must not pose a “direct threat” to the health or safety of the individual or others in the workplace or in the job performance.
DESCRIPTION:
The skilled Nurse has a clinical nursing knowledge, physical assessment, teaching and procedural skills. He or She has the ability to be directly responsible for assessing planning, providing and documenting nursing care for a designated group patient in accordance with the physician’s plan of care and MID AMERICA HOME HEALTH CARE, INC.’S policies and procedures. The provisions of quality of care is accomplished by implementing, supervising and coordinating services provided and utilizing division to LPN’s, Home Health Aides and completes documentation effectively and timely.
DUTIES AND RESPONSIBILITIES:
- Completes referrals and takes physician’s verbal orders as necessary in a timely manner. Channels referrals appropriately to Agency nursing supervisor for assignment.
- Makes home visits, performs physical assessments, evaluates patient’s needs and consults with physicians if necessary to develop plan of care and documents appropriately and in a timely manner.
- Assess needs to additional health or ancillary services and refers patients for service(s) when appropriately, on a timely basis.
- Assess family and home environment and make recommendations, which will enhance patient care and promote safety.
- Involves patients and or family in developing and revising an individualized care plan including necessary teachings with realistic goals, update plan of care as goals are met or change in intervention as required and documents changes appropriately within five (5) days.
- Coordinates the multidisciplinary care team and plan for patient care and documents appropriately.
- Interprets the admission, discharge and other policies of the company to patient, physician, and family, other health care providers and community as needed.
- Providers, clear written and verbal Home Health Aide and LPN assignments, updates instructions for the Home Health Aides and LPN’s in a timely manner and documents instructions appropriately.
- Provides, skilled nursing care and assessments to patients in the home environments, according to the plan of care an promptly revise the plan, as necessary, in consultation with the physician within four (4) hours of changes in patient’s condition and document communication with physician appropriately.
- Informs physician within four (4) hours of changes in patient’s condition and document communication with physician appropriately.
- Initiate appropriate preventive, rehabilitative, comfort, pain management, nursing procedures and provides instructions to patients/caregivers and staff in timely manner. Follows company policies and utilizes company forms for proper documentations.
- Explains to the patient and family the diagnosis and the nature of the treatment consistent with the physician’s orders and plan of treatment.
- Performs LPN supervisory visits to evaluate competency, at least once every month or more frequently, if necessary. Provides skill services and appropriate instructions to LPN and documents accordingly.
- Conduct Home Health Aide supervisory visits at least every two weeks either when the Home Health Aide is present to observe and assist or when the Home Health Aide is absent to assess relationship and determine whether goals are being met.
- Perform specialized care in accordance with the competencies and demonstrate initiative in learning new skills and developing competency.
- Maintains assigned patient’s clinical records current including Medication Profiles, Plan of Care, Case Conferences, Progress Notes and other required documentation. Conducts patient care conferences (PT, OT, and SLP) every three weeks from the start of care and report unusual occurrences / changes to the attention of the physician. The patient and family are consulted and informed about all changes in the care of plan.
- Completes clear, concise and accurate clinical notes which documents skilled services and other required documentations appropriately and submit on a timely basis. Utilizes only universally approved documents.
- Communicates necessary information to the Nursing Supervisor.
- Demonstrates professionalism by maintaining high standards of clinical care and staying current on clinical and medication issues.
- Attend and participates in multidisciplinary patient care conference to demonstrate effective coordination and continuity of patient care, as well as, schedule care to be brought to the clinical records review committee.
- Conducts and documents formal and informal case conferences to demonstrate effective coordination and continuity of patient care, as well as, schedule case to be brought to the clinical records review committee.
- Works with patient, family and health care team members effectively to coordinate services and to promote quality care and positive patient care outcomes.
- Cooperates and is reliable with on-call coverage and making PRN, weekend and holiday visits to the patients.
- Establishes and maintains working relationships and open lines of communication with personnel from physician’s offices are hospitals, clinics, ect. to assure flow on information, referrals for services and continuity.
- Completes nutritional assessments and provides patient/caregiver instructions on diet/ nutrition and seeks consultation as necessary.
- Uses appropriate teaching materials and technique to obtain positive outcome and seek consultation as necessary and documents patient/ caregiver outcomes on Agency forms.
- Uses appropriate infection control procedures and safety measures.
- Administer CPR, provides emergency care necessary and maintains current CPR certifications.
- Assist in orienting or as a Preceptor to new staff when necessary.
- Provides and documents services in accordance with third party payer requirements and obtains prior authorization as required. Submit 485/recertification information timely, accurately and legibly. Notifies date processing of payer source. Changes in timely manner.
- Provides and documents services in accordance with Federal State, Medicare and Medicaid regulations as applicable.
- Performs job duties and uses resources appropriately.
- Coordinate with Administrator/ Agency nursing supervisor to improve deficient services and the development and implementation of plan of correction.
- Address policies and procedures of Mid America Home Health Care, Inc.
- Performs other duties as assigned by the Agency nursing supervisor.
Position requires traveling in all weather conditions, as well as, working in homes of all socioeconomic levels. Exposure to communicable diseases, infections, dust, tobacco smoke and animal dander may also occur
