The Midnight Mission is the oldest continuously operating human service organization in the Los Angeles region. Centered in the Skid Row area of downtown Los Angeles, The Midnight runs one of the most efficient direct service operations in the country. With only five executive managers throughout its 100 year history, The Midnight has been a consistent beacon of light for those with nowhere else to turn.

We are currently seeking a Case Manager, who, at the direction of the Director of Programs, assumes responsibility for counseling, crisis intervention, client outreach, intake, and case management of program participants.

Key Duties and Responsibilities

•    Oversees and guides the rehabilitation and transition plans for Program Participants.
•    Conducts comprehensive screenings and assessments with individuals to collect functional, environmental, psycho-social, financial, employment, housing, educational, and health information, as appropriate, to develop an Individualized Service Plan that identifies priorities, desired outcomes, and the strategies and resources to be used in attaining the outcomes.
•    Following Assessment, prepare with the client a plan that is a roadmap of services needed, actions to be taken and referrals to be made.
•    Conducts housing focused case management sessions dedicated to assessing and reassessing needs, educating participants, monitoring progress of plans towards resolution of the participant’s housing crisis. Maintain HMIS data on participant, report progress monthly.
•    Provide service linkages and support systems to ensure identified needs are met, coordinate with outside service providers.
•    Prepares and updates case file notes and reports on program clients’ progress
•    Conducts program intake screening interviews, and recommends program placement or referrals
•    Moderates discussions and workshops on anger management, conflict resolution, and life planning skills.
•    Provide respectful engagement to adults with histories of chronic homelessness, serious psychiatric disabilities and co-occurring substance use disorders.
•    Work with other partners to develop strategies that help encourage homeless individuals to take steps leading ultimately to housing.
•    Lead service coordination for clients including assuming primary responsibility for developing and implementing goals and plans for each client in collaboration with the client and the team, providing clients with support, guidance and encouragement on their journey to housing.
•    Proactively engage clients on the street, trying out-of-the-box ways of reaching clients who have been living on the street for many years and who may have significant trust, mental health and substance use issues.
•    Act as liaison to and consult with partners and community agencies and programs to maintain coordination in the treatment process. 
•    Help with paperwork needed to place clients with vouchers into housing. 
•    Help clients adjust to life in housing, including but not limited to engaging clients around goals and aspirations, life skills, hobbies, and other activities that may be needed during the adjustment to housing.
•    Complete various special projects as directed.
•    Produce various reports for staff, management and board of directors.
 

Qualifications


Education/Requirements
•    High School diploma, or equivalent experience required, AA preferred.
•    Three years (3) experience in mental health treatment or related social service experience. (preferred)
•    Valid Calif. Drivers License

PHYSICAL REQUIREMENTS

Sedentary work. Exerting up to 10 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Sedentary work involves sitting most of the time.
