Under the supervision of the HIV Services Director and Social Services Coordinator, and in conjunction with the HIV multi-disciplinary team, the Recruitment and Retention Coordinator (RRC) is responsible for maintaining and evaluating patient retention strategies and tracking systems; providing linkage to care and engagement via case management and supportive counseling to newly-diagnosed HIV-positive individuals or HIV-positive persons out-of-care. 

 

Essential Functions and Responsibilities:

The HIV Recruitment and Retention Coordinator has the following responsibilities and duties:

  • Maintain database systems to identify and track clients out-of-care or tenuously in-care. Work with the Treatment Link Specialist, Outreach Workers and Case Managers to engage clients in care.
  • Coordinate and monitor the RAPID linkage protocol for new diagnoses, including data tracking and intensive case management.
  • Assess clinic procedures to identify any barriers to care, as well as propose policy and procedure updates to streamline access to care for clients.
  • Coordinate grant projects and campaigns specific to linkage, engagement and retention.
  • Coordinate relevant staff, including the Treatment Link Specialist, to enhance linkages to care and engagement, and improve data collection and tracking. Ensure services are not duplicated and all clients are linked to care and/or have a follow-up plan.
  • Coordinate with the SFDPH LINCS program and community HIV testing sites to ensure rapid linkage to care for newly diagnosed individuals.
  • Enhance and evaluate the CE patient recruitment strategy; including the development of marketing tools to enhance recruitment of new clients; build community partnerships and organizational outreach to expand MNHC's profile and recognition as the premier provider of HIV services for Spanish-speaking Latinos and other under-served clients with multiple needs. Work closely with the HIV Outreach team to build our public profit and distribute marketing materials and information.
  • Manage Social Media sites, create content, and analyze engagement metrics to identify ways to maximize reach.
  • Monitor patient referrals and hospitalizations to ensure successful access and prevent re-admission.
  • Attend weekly multi-disciplinary team meetings and present treatment readiness assessment and service plan when warranted. Participate in staff meeting and in-service training. Receive bi-weekly clinical and administrative supervision.
  • Submit monthly activity reports and statistical data to the HIV Services Director.
  • Other duties as assigned.
  • Work with "hard to reach populations" in public settings to facilitate access for HIV positive individuals to early intervention, primary care and other support services
  • Serve as the first contact to Clínica Esperanza for potential clients identified through external referrals, self-referred walk-ins and previously identified out of care individuals. Conduct a screening and brief readiness assessment with these clients; act as a liaison for Clínica Esperanza and other agencies; and facilitate referrals as necessary for newly diagnosed or out-of-care HIV positive clients.
  • Conduct surveys and assessments with eligible clients and provide treatment education and supportive counseling to at-risk HIV-positive individuals who are out of care.
  • Be knowledgeable of medical services available in San Francisco County and develop specific protocols for identifying and reaching HIV persons of color.
  • Empower and educate clients to make informed decisions regarding their utilization of health care services.
  • Maintain a relationship with all clients after linkage to care, through periodic follow-up defined by patient needs and consultation with the SSC.
  • Documentation of interventions in client medical records; documenting clients' profiles and record for reporting, referrals, and networking.

Qualifications & Requirements

The HIV Recruitment and Retention Coordinator should have the following skills, education, and experience:

  • Bachelor's degree in related field. MSW, MPH or Masters in related field preferred
  • Ability to communicate effectively with target patient populations and community partners.
  • Documented experience with data system development and maintenance, monitoring and evaluation.
  • Experience with marketing and public relations and/or outreach.
  • Experience in program management/coordination preferred.
  • Ability to work professionally and ethically within multi-cultural settings, including ability to maintain confidentiality and privacy of persons, documents, data, and communications.
  • Sensitivity to HIV/AIDS and Gay, Lesbian, bisexual, and Transgender issues.
  • Bi-lingual English/Spanish required.
