PATIENT NAVIGATOR FOR AMBULATORY SAFETY NET (ASN) PROGRAM, MIT Medical, to ensure a seamless patient experience by integrating all components of a patient’s medical plan and facilitating direct access to care. Will work under the supervision of the program manager and population health coordinator and enable the coordination of multidisciplinary follow-up care for patients enrolled in the ASN patient registries. Responsibilities include engaging and creating a trusting relationship with patients and their families and acting as their primary point of contact; streamlining the healthcare process for patients and their families by facilitating follow-up clinic appointments and providing necessary loop closure with consulting services; working to eliminate barriers to care by identifying critical resources for patients and helping them navigate through health care services and systems to prevent missed and delayed diagnoses; working closely with the care team to support positive patient health outcomes; coordinating proactive patient outreach to schedule follow-up appointments by working with the referral office staff; coaching and educating patients as they prepare for cancer screening procedures and providing reminders about their pre-visit lab tests; and providing continual support beginning with the initial consultation and continuing through diagnosis to treatment and follow-up care.
Job Requirements
REQUIRED: a high school diploma or its equivalent; at least three years’ experience in a healthcare environment; experience in social work, health promotion, public health, or patient care coordination; and training or experience in population health management.
