Summary: The primary purpose of this position is to coordinate a client-centered approach in the identification, development, and implementation of all services and supports for each individual during his/her stay at Minnesota Adult & Teen Challenge. This position will assure that appropriate outcomes are identified and developed through the Treatment Team process, that progress is monitored, and that plan revisions are made based on the client’s needs on an on-going basis.
Treatment Coordination is defined at 245G.07, subdivision 1, paragraph (a), and clause (6) and includes:

Assistance in coordination with significant others to help in the treatment planning process whenever possible;

Assistance in coordination with and follow-up for medical services as identified in the treatment plan;
-
Facilitation of referrals to substance use disorder services as indicated by the client’s medical provider, comprehensive assessment, or treatment plan;
-
Facilitation of referrals to mental health services as identified by a client’s comprehensive assessment or treatment plan;
-
Assistance with referrals to economic assistance, social services, housing resources, and prenatal care according to the client’s needs;

Life skills advocacy and support accessing treatment follow-up, disease management, and education services, including referral and linkages to long-term services and supports as needed, and
-
Documentation of the provision of SUD care coordination services in the client’s file.
Essential duties and responsibilities as it pertains to MnTC employment:
Coordination of Care with Client Care Team:
-
Aid in the successful entry of clients into the Long-Term Treatment Program by meeting with clients individually to ensure clients’ needs are being met in accordance to their treatment plan.

Collaboration with care team members for the purpose of treatment coordination follow-up.

Attend 1 hour of monthly supervision with LADC.
Coordination of Aftercare Services:

Conduct meetings with clients who desire to exit the Long-Term Treatment Program prematurely.

Working with care team and community team members, develop an exit plan for clients who are exiting the Long-Term Treatment Program at staff request and coordinate transfer of client to outside, community facilities.

Maintain working knowledge of community resources including, but not limited to: housing, social services, educational services, support groups, faith based services, and legal services.

Aid in families involvement as client transitions through Aftercare services; helping clients set up appropriate support systems.

Meet individually with clients to determine needs and facilitate resources; helping clients in their transition from the Long-Term Treatment Program upon graduation into the community.

Hold meetings with clients to establish aftercare plans and maintain appropriate charting to communicate clients’ aftercare plans to various care team members.

Hold meetings with clients for purposes of teaching medication and appointment coordination skills.
Data Management and Coordination:

Collect and audit medication refusal data while communicating with client and client care teams to ensure adequate follow-up.

Minimum Qualifications:

has a bachelor's degree in one of the behavioral sciences or related fields

Must be free of problematic substance use for 2 years prior to date of hire.
