The Lead Health Coach will maintain Health Coaching caseload and provide training and guidance to the Health Coaching team. The Lead Health Coach will develop and implement behavioral-based member and provider facing coaching programs and health care self-management support, including condition-specific disease management health coaching and maternity care to our members. Visits will address issues of care surrounding co-morbid chronic disease in conjunction with recent out/in-patient transitions of care. This position will collaborate with all Healthcare Services and Pharmacy staff to ensure integration with other programs. The Lead Health Coach will interact with the member, family, and care providers to develop, coordinate, and monitor the member’s treatment plan.
- Current Oregon license as, Registered Dietician (RD), Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), Registered Physical Therapist (RPT) or two years relevant chronic care management and/or health coaching experience.
- At least 4 years of experience in disease management/health coaching program with health plan or similar organization is strongly preferred. Training in motivational interviewing is preferred.
- Previous experience with asthma, COPD, diabetes, cardiac, congestive heart failure, medication therapy management, severe obesity, musculoskeletal disorders, or depression education programs preferred.
- Previous experience as a certified diabetes or asthma educator is preferred.
- Strong organizational and time-management skills.
- Ability to problem-solve, recall information, and attend to details.
- Ability to speak multiple languages preferred.
- Strong verbal, written and interpersonal communication skills.
- Proficiency in Microsoft Word, Excel, Outlook, and Access (data entry forms). Ability to learn proprietary databases.
- Ability to type a minimum of 35 wpm on a computer keyboard.
- Valid Oregon driver’s license. Ability to handle a push cart/hand truck and to lift boxes up to 40 pounds in and out of a car trunk.
- Ability to maintain confidentiality and project a professional business image telephonically and in person.
- Oversee Health Coaching practices of the Health Coaching team
- Assist Supervisor with training schedules and materials development for new Health Coaches as well as identifying and implementing training activities for current staff.
- Monitor training progress of new Health Coaches
- Lead professional development activities related to behavior based coaching practice (MI, PAM, etc.)
- Audit cases in Dynamo for compliance with Health Coaching policies and procedures
- Maintain established Health Coaching programs.
- Provide initial and ongoing assessment of member needs.
- Identify member’s personal healthcare goals.
- Create and maintain customized action plan.
- Support self-management activities and achievement of goals through phone, mail e-mail and in person interventions, which include motivational interviewing.
- Coordinate and contribute to continuous quality improvement efforts related to coaching programs, including review of evidence-based guidelines, review of new resources and participation in work groups.
- Utilize customized database systems for consistent and accurate documentation of member contacts, status, and outcomes.
- Consult with healthcare analyst, health promotion & quality improvement specialist to support data collection and quality improvement efforts. Provide clinical input and coding expertise.
- Communicate with providers and office staff regarding ongoing plan of care and patient progress towards goals. Develop partnerships and encourage program participation.
- Participate in employer and community-based health fairs, providing basic health screenings, related counseling and educational materials to members and the public.
- Contribute to the selection, development, implementation and evaluation of educational materials and other member communications.
- Maintain professional relationships with other Moda Health staff & departments.
- Perform other duties as assigned.
