JOB SUMMARY
The perinatal nurse navigator is a registered nurse dedicated to providing comprehensive management of patients throughout pregnancy and postpartum; acting as a liaison to enhance patient engagement, quality, and continuity of care. He/she functions as an advocate, educator, and care coordinator by guiding and assisting patients and families with their concerns, education and healthcare needs from the first prenatal visit through the first year postpartum focusing on health promotion, disease prevention, and optimal mother/baby outcomes.
The perinatal nurse navigator establishes and maintains collaborative relationships with physicians, health care providers, community resources, patients and their families to achieve the desired patient outcomes and provide support throughout the continuum of care. The perinatal nurse navigator serves to align OB education, policies and workflows at MHMC, SJMH and PMH. The perinatal nurse navigator will focus on P3 program development, research/data collection and seek opportunities for grants to enhance program growth. In addition, the perinatal nurse navigator is responsible for patient education, staff education and maintenance related to our baby friendly designation.
MINIMUM QUALIFICATIONS
Education:
Graduate of an accredited school of nursing. Preferred: BSN.
Experience:
Minimum of 2- years labor & delivery, postpartum and nursery experience.
Licensure/Certification:
WV RN License; BLS
CLC within 1 year. PSI Certification within 1 year.
PHYSICAL REQUIREMENTS & ENVIRONMENTAL CONDITIONS
Light Work: Frequent lifting and/or carrying of objects weighing up to 10 lbs. with a maximum lifting of 25 lbs.

Salary : $66,600 - $84,300
