What are the responsibilities and job description for the Care Manager Assistant position at MonmouthCares?
MonmouthCares (http://www.monmouthcares.org) is currently seeking a Care Manager Assistant to provide administrative support to Care Management operations.
Responsibilities of this position include:
- Manage daily enrollment of youth and families.
- Maintains Provider Management log for all Mobile Response referred youth.
- Facilitates the Continuity to Care (C2C) - for identified children entering out of home (OOH) treatment.
- Obtain necessary evaluations and assessments as requested by Care Managers for treatment planning.
- Participates in Care Management trainings, CM team and staff meetings.
- Assists families with completing the Intellectual/Developmental Disabilities (I/DD) application for DD eligibility.
- Completes monthly Peer Review/Quality calls.
- Facilitates the ISP Distribution to system partners (Division of Child Protection and Permanency, Probation)
- Executes the process for safeguarding the Digital Designated Record Set (DDRS) releases.
- Oversees the Para Plus process for translation services and communicates final invoice with internal stakeholders for payment by Accounts Payable/Business Department.
- Completes Youth Link Report for youth entering Out of Home Treatment
- Qualifications:
- BA/BS in a relevant field ((e.g., social work, counseling, psychology, psychiatric nursing, criminal justice, special education)
- Knowledge of community resources
- Language: bi-lingual (English & Spanish, Portuguese and/or Creole) a plus
- A valid New Jersey driver's license is required.
