What are the responsibilities and job description for the QMHP-C Counselor position at More Solutions Youth and Family Services?
- Manages a small caseload of children, adolescence or adults, providing services on an intensive basis and utilizing short-term, solution focused treatment strategies.
- Facilitates wraparound team meetings to enlist existing supports in determining and meeting service objectives and to identify additional resources and supports.
- Prepares written service plan, incorporating input from clients, involved service providers, and wraparound team members, and clearly identifying problem areas and needs, strategies, and service objectives.
- Provides direct clinical services to children, including individual, family, couple, and group therapy, and issue-specific or educational groups (parenting skills, anger management, substance abuse interventions).
- Responds to crisis situations with twenty-four-hour-a-day availability.
- Scheduling and transporting clients to Medical, Psychological, Psychiatric appointments
- Assessing needs for physical resources, equipment, and supplies
- Teaching and developing skills of daily living (i.e. hygiene, budgeting, food shopping and preparation)
- Identifying and establishing professional relationship with community resources
- Advocating on behalf of clients while teaching clients self-advocacy skills
- Other duties as identified and/or assigned
- Prepares progress reports as required by referral source or service purchaser.
- Participates in Child Specific Team, Family Assessment and Planning Team, and other interdisciplinary, diagnostic, or planning meetings.
- Collaborates and coordinates with other case involved service providers and professionals, such as attorneys, school personnel, social workers, and probation counselors.
- Links clients with external programs or services, such as health services, recreational activities, child care services, financial resources, employment resources, child care, AA/NA groups, transportation resources, and others.
- Attends court hearings when families are court-involved and presents testimony and treatment recommendations as requested by attorneys, referral source, the court, or clients.
- Works with Clinical Supervisor of work load, case activity, service barriers, and procedural problems in staff and one-on-one meetings.
- Professional training 20 hours per year required. Training hours can include but are not limited to the following: Documentation of orientation, 6 weeks of individual (clinical), 4 weeks of group, administrative orientation, in-service training, and trainings at other places of employment. Classes taken should all be documented on training form. All other trainings must be approved by Director.
