MorseLife Health System SUMMARY
Responsible for the coordination of patient visits, maintenance of scheduling records, and accurate and timely communication of schedule changes to the office and field staff. Maintains accurate data in Soneto by verifying all visits are completed 100% within 24 hours.
Primary Functions
Coordinates and schedules all field staff assignments, making certain that all patients are assigned to staff that are qualified to provide the level of care, as determined by the RN or PT assessment and with consistency to physician orders.
Communicates directly with field staff on a daily basis and documents confirmation and/ or changes in scheduling software.
Schedules new cases to ensure care coordination with nursing and rehab are within two days of each other.
Updates patient status in Soneto immediately for pending, hospital holds, non-admit, MOD orders, etc.
Notifies the nursing supervisor any time an employee with the required qualifications is not available as needed.
Logs in all OASIS and therapy evaluations immediately after receipt and tracks OASIS within 48 hours.
Serves as the liaison between referral sources and MorseLife Homecare through the referral intake process, including ongoing and immediate communication with marketers; patient and/or patient caregivers; ordering physician; and other home health agencies (if applicable) to obtain all required and accurate information to enter into Soneto.
Verifies time sheets and visit notes and enters computerized data for creation of payroll.
Provides on call services when necessary
Maintains current client roster and patient reports for management review. Prepares nursing missed visit reports for management review once a week.
Oversees Accountants Receivable process for all MorseLife Homecare agencies as needed.
SUPERVISORY RESPONSIBILITIES
This position may include some supervisory responsibilities
QUALIFICATIONS (EDUCATION and/or EXPERIENCE)
High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.
