Mozaic (a chapter of the Arc) is looking currently looking for a highly organized and detail-oriented person to join our team as a Medical Coordinator!
GENERAL DESCRIPTION
- Responsible, in conjunction with the Residential RN, IRA Managers and Program Nurse, for the coordination of medical appointments for Individuals receiving services in the Residential Program. This includes, but is not limited to, scheduling, transportation, documentation, and any necessary follow-up.
- Provides additional support assisting Individuals receiving services in training of activities of daily living, including providing opportunities for integration in the community, socially and recreationally.
- May provide day-to-day coverage at various residential sites as needed.
MINIMUM QUALIFICATIONS
- LPN is preferred with one-year experience in human service field; or Associate's degree with two years experience in human service field.
- Valid NYS driver's license
