What are the responsibilities and job description for the Health Coach position at Mt Juliet Health & Wellness?
We are looking for a health coach to work with patients in the Mt Juliet, TN. This is a part-time position but will increase to a full-time position, with an average of 20 hours per week initially.

The ideal candidate has a strong background in health and wellness, excellent communication skills, and a genuine desire to help others improve their overall well-being.
This position requires patient interaction, so it is important that you have the ability to think on your feet and be very adaptable. You will be working with patients on a wide variety of topics, so you will need to be flexible and able to handle different tasks.

Tasks
- Educate clients on healthy lifestyle choices, including nutrition, exercise, and stress management.
- Monitor patient progress and assist with making adjustments to their plan as needed
- Collaborate with provider to provide comprehensive care of patients.

Duties: (include but not limited to)
- Work with patients in person, via phone, or via Zoom video conference. This includes walking, standing, sitting, kneeling, bending, and other activities that require physical contact with the public.
- Provide patient follow-up and document all visits, track progress and results, and make modifications as needed.
- Provider services for bio-idential hormone replacment therapy (BHRT) program. Under the direction of a licensed clinician, perform medical procedures including EKG's, vital signs, autoclaving and provision of sterile instruments, for pellet injections.
- Assist in daily clinic operations, including opening and closing of procedures, and maintining a tidy environment.
