Multicultural Home Care is seeking a bilingual RN to provide quality nursing care and related services in the home.
RESPONSIBILITIES AND FUNCTIONS:
1. Provides quality nursing care and related services in the home.
2. Maintains and demonstrates effective client services in an accurate and timely manner.
3. Adheres to the agency’s policies and procedures as required.
4. Performs related accountabilities/ responsibilities as required or directed.
5. Provides those services requiring skills using the Nursing Process (Assess, Plan, Implement, and Evaluate.)
6. Continually evaluates the client’s nursing needs and works with the client/caregiver in developing ways to meet their needs.
7. Follows a current, individualized plan of treatment by making revisions based on original and ongoing assessments and in conjunction with the doctor, case manager, and client/caregiver.
8. Informs the physician, case manager, and other healthcare team members of changes in the client’/s condition and needs.
9. Initiates and performs teaching of the client, family, and other health team members, as appropriate, in order to meet identified needs and achieve goals.
