Summary/Primary Purpose: Provide community-based supportive service for very-low income Veteran families residing in or transitioning to permanent housing. The Case Managers will provide supportive services to individuals to maintain community living.
Essential Duties and Responsibilities:
- Complete comprehensive strength-based individual and family needs assessment within 3 days of referral.
- Collect information and develop Housing Plan, integrating individual’s strengths and needs, considering community, family and other supports, suggesting measurable goals and specific interventions that include documented discharge plans within 7 days of admission.
- Within 7 days of admission and updating on a regular basis, individual service plans to address the areas of: increasing income (VA benefits and public entitlements or employment), greater self-determination (something to accomplish for themselves) and strategies to ensure individuals maintain residential stability.
- Provide direct active support interventions with focus on: Activities of Daily Living, education, budgeting, problem solving, crisis intervention, social skills, and independent living skills.
- Perform services coordination and case management activities including transportation, assistance in accessing medical, social education, housing, transportation or other appropriate support services.
- Assist participant with obtaining a credit report and developing a debt repayment plan to review progress monthly.
- Present program participants with presenting problem at weekly supervision, and monthly to Multi-Disciplinary Team to obtain feedback to address any barriers or clinical issues.
- Assist in maintaining case records, compiling statistics and writing reports and manuals as requested by the Program Coordinator.
- Complete all required case file documentation including but not limited to ISPs, case notes, intake and discharge paperwork, needs assessments, and entering participant information in HMIS system.
- Attend and participate in MACCH general membership meetings and task forces.
- Provide up to three months of voluntary aftercare services
- Provide on call coverage rotation including taking calls via telephone and going into the office to address a situation if warranted.
- Complete 15 hours of intensive on-going training per year based on program/participant population need including but not limited to: Trauma Informed Care, Co-Occurring Disorders, Rent Wise, Fair Housing, SSI/SSDI Outreach Access and Recovery (SOAR), Supplement Nutrition Assistance Program (SNAP), and Trauma Informed Care.
- Driving is an essential function of the position.
