Job Description Summary
The (DDC) Digestive Disease Center Program Coordinator l reports to the (DDC) Front End Operations Manager. Under general supervision, the Program Coordinator assists in referral management, requesting medical records, managing incoming records, scheduling and communicating with patients. This role helps to establish, coordinate and perform administrative duties required to support the RN Coordinators. This position operates as the liaison between patient and clinical staff in communicating relevant information by telephone and written correspondence.Entity

Job Description
1. Provide administrative support for RN Specialty Coordinators
- Responsible for maintenance of Fax Hub.
- Obtain patient medical records, images, and lab results from outside facilities as needed.
- Coordinate with the Administrative Assistant to manage office supplies/records.
- Responsible for scanning records into EPIC.
2. Coordinate all new patient referrals and clinic appointments into EPIC (self-referrals, internal and external physician referrals)
- Maintain receipt and distribution of faxed records through Fax Hub.
- Communicate referral receipt to RN Specialty Coordinators.
- Create referrals in Epic.
- Upload and scan medical records into Epic via Solarity Scan.
- Upload radiology images into LifeImages.
- Work with RN Specialty Coordinators to schedule patients for clinic appointments as needed.
- Answer all phone calls from various referral sources.
3. Promotes departmental communication to other departments and patients
- Personally, handles applicable mail.
- Independently organizes and compiles documents as requested by Coordinators.
- Performs typing in an accurate and professional manner.
- Participates in activities to maintain and/or enhance the competency in administrative and clerical duties
4. Participates in appropriate continuing education.
- Keeps current with changes with MUHA processes and training.
- Attendance at staff meetings as required.
- Reads and responds to emails daily.
- Obtains education related to new information technology systems and
- their administration.
