What the Clinical Care Lead Does
- Provides direct supervision to Nurse Practitioners and Physician Assistants.
- Leads organizational efforts to ensure timely completion of deliverables assigned to the business unit.
- Serves as a representative for My Choice Wisconsin at external workgroups and meetings.
- Coordinates and facilitates discipline meetings including developing agendas and arranging speakers.
- Assists with providing guidance, expertise, and training to staff on processes and procedures, related to program, case management, and/or discipline specific issues and organizational contract requirements.
- Provides input to manager regarding staff performance.
- Precepts and provides orientation and training to new hires and/or potential candidates including scheduling shadows/visits.
- As assigned, maintain a small workload that may consist of any of the following, but is not limited to the below list:
- Conduct comprehensive history and physical assessments consistent with department process. Identify all active and chronic disease conditions as well as determine all physical, mental, and social needs present at the time of the visit. Obtain vital signs, measure BMI, review pharmacological therapy and conduct a physical examination.
- Capture and document annual diagnoses accurately consistent with Medicare risk adjustment.
- Conduct comprehensive annual medication reviews and follow up as necessary.
- Identify gaps in acute/primary care and chronic disease management.
- Identify medical risk and collaborate with members of the care team to mitigate.
- Communicate recommendations and collaborate with primary care providers/specialists to ensure clinically relevant treatments are explored, if not already in place.
- Communicate recommendations and collaborate with other providers to confirm suspected, but not yet identified/coded, diagnoses.
- Develop a medical plan of care in conjunction with primary care provider/specialists as dictated by member need and assessment findings.
- Collaborate with the Manager of Clinical Care and other clinicians in the department to assure effective patient care consistent with quality indicators for patients cared for in the designated service.
- Educate and counsel member and family on any conditions identified during assessment and screening procedures.
- Provide quality member centered care.
- Ensure timely appointments and documentation per department procedure.
- Provide consultation, as needed, for medically complex members, high utilization or a subject-matter expertise (e.g. Palliative Care, Dementia, Medical, Psychiatric).
- Other duties as assigned.
Minimum Qualifications:
- Nurse Practitioner:
- Graduate of an accredited school of nursing.
- Graduate of an accredited Nurse Practitioner Program
- Current national certification as Nurse Practitioner through ANCC or AANP in Geriatrics, Adult-Geriatric, or Family practice or eligibility to obtain within reasonable timeframe.
- Current and unrestricted State of Wisconsin Advanced Practice Nurse Prescriber license and Drug Enforcement Administration registration required.
- Physician Assistant:
- Bachelor's or Master’s degree in Physician Assistant studies of an accredited school.
- Current State certification as a PA, or eligibility to obtain within reasonable timeframe, current and unrestricted State of Wisconsin Physician Assistant license and Drug Enforcement Administration registration required.
- Work in conjunction with My Choice Wisconsin medical director.
- A valid Wisconsin Driver’s License, access to a reliable vehicle, and automobile insurance coverage at the time of application throughout employment.
Preferred Qualifications:
- Previous experience as a Registered Nurse and/or Nurse Practitioner/Physician Assistant as well as experience with the geriatric and/or disabled populations.
- Three or more years of clinical experience.
- Previous supervisory and/or leadership experience.
