What are the responsibilities and job description for the School Based LPN position at Nationwide Children's Hospital?

Overview:
This is a FTE 0.8, working 32 hours per week at KIPP Academy
Minimium pay rate: $20/hr

The LPN provides technical nursing care to acute and chronically ill children from the age of birth to 21 years and selected adults patients, under the direction of a RN Care Manager, and in accordance with the Ohio Board of Nursing Law and Rule. (Sections 4723-4-01, Ohio Administrative Code or OAC, 4723-4-04, OAC, 4723-4-06, OAC, and 4723-4-08 OAC). Demonstrates competence defined (*) in LPN II responsibilities in providing age appropriate care and service to pediatric patients from birth to 21 years, and selected adults served within the school setting.

Responsibilities:
1. Follows the plan of care appropriate to the developmental level of the patient. Collaborates with the NCH APN directing the patient’s care regarding appropriate tasks/nursing interventions.
2. Implements nursing interventions according to established standards demonstrating competence in specified nursing care. Administers and documents medications by all routes, i.e. oral, subcutaneous, intramuscular, intradermal, transdermal, rectal and aerosol, utilizing the five rights and all other related safety recommendations.
3. Registers patients for visits and accepts referral from school nurse and communicates to APN.
4. Works with school staff to locate students for appointments. Works in accordance with building protocol to provide passes to students missing class for appointments. Escorts students back to class when necessary.
5. Transports lab specimens and immunizations from clinic to lab, when necessary.
6. Collaborates with members of the quality improvement team, NCH school health staff and school personnel to track metrics of success for school health services. Researches and compiles data for reports and tables.
7. In collaboration with leadership, establishes effective vehicles of communication across internal and external parties involved in school health related initiatives. Communicates problems and resolutions to NCH APN and NCH school health liaison.
8. Establishes and maintains appropriate departmental records, procedures and documentation. Assists in the development of a procedural manual as team members define standards and processes.
9. Promotes the organization and the clinic to the community and school. This may require occasional attendance at night, evening and weekend events at the school.
Qualifications:
Education: Must be a graduate of an approved school of practical or vocational nursing.
Licensure: Must be licensed as a Practical Nurse in the State of Ohio. Valid Ohio driver’s license and transportation required to travel to off site facilities; proof of auto insurance required.
Certifications: Must complete an Ohio Board of Nursing approved medication test, such as NAPNES (National Association for Practical Nurse Education and Service) or OAPNES (Ohio Association for Practical Nurse Education Service.) Must satisfactorily complete orientation objectives as determined by the manager.
Skills: Basic written and verbal communication skills. Demonstrated competence in specified nursing care. Positive, effective, and age appropriate interaction skills with patient/family, physicians and other health care team members
