Role Description
This is a part-time hybrid role for a Hospice and Palliative Care Social Worker at Nationwide Children's Hospital located in Columbus, OH. Schedule is 8-4:30 Tuesday-Friday. In this role, you will be responsible for providing excellent pediatric care to patients and families in a 9-county, Central Ohio region, in the patient homes. Your day-to-day tasks will include conducting assessments, offering emotional support and presence, and connecting families with community resources. This role meets patients and families at admission and can follow through bereavement. This team consists of seven disciplines working very collaboratively.
Qualifications
- Strong communication and interpersonal skills
- Experience in providing counseling and support to patients and families
- Knowledge of community resources and social service programs
- Excellent organizational and time management skills
- Master's degree in Social Work
- Ohio Licensure as a Social Worker
- Experience working with pediatric patients is a plus
- Experience working with hospice is a plus
