What are the responsibilities and job description for the Nurse Practitioner position at NC Elder Care?

The Nurse Practitioner with NC Elder Care will provide primary care to patients in various long term care settings and collaborate with facility staff to deliver high quality care on site. Fostering relationships with the facility staff and primary care/specialty providers in the community, providing educating, and demonstrating clinical leadership are some of the tangible ways the nurse practitioner provider will help to address the complex needs of the patients and families. There is flexibility and autonomy in this role. Making a positive impact in the care of our older adult clients is our goal. Role and Responsibilities

  • Provide primary care for patients in assigned long term care facilities
  • Work collaboratively with primary care/specialty providers to provide the best care possible
  • Work collaboratively with facility staff and families
  • Provide clear medical documentation of care provided, either written or within the EHR
  • May be required to participate in on-call/after-hours call*

Qualifications and Education Requirements

  • Certified Nurse Practitioner (ANP, GNP, FNP, AGNP) through a national board (ANCC, AANP)
  • Minimum of master’s degree in Nursing
  • Current unrestricted license (RN & NP) or the ability to obtain in state(s) in which practicing
  • Unrestricted DEA (license to prescribe narcotics / level II info here) per state regulations (unless prohibited in state of practice) or the ability to obtain
  • Access to reliable transportation that will enable you to travel to client and / or patient sites within a designated area

Preferred Skills

  • Long-term care experience in a variety of settings (SNF, ALF, group homes, CCRC, etc)
  • Proficient with computer systems
  • Understanding of geriatrics, chronic illness, and advanced/terminal illnesses.

Job Types: Full-time, Contract

Pay: From $85,000.00 per year
