Summary:

The LPN in home health provides direct patient care in the patient's home under the supervision of a registered nurse (RN) or physician. This role involves assessing patients' health, implementing care plans, and working with patients and their families to manage health conditions and achieve optimal health outcomes.

Responsibilities:

• Administer medications and treatments as prescribed by the physician.
• Monitor and record patient vital signs, including blood pressure, temperature, and pulse.
• Assist with daily living activities such as bathing, dressing, and grooming.
• Provide wound care and manage medical equipment.
• Perform routine physical assessments and report any changes in patient condition.
• Develop and implement individualized care plans in collaboration with the RN, physician, and other healthcare professionals.
• Educate patients and their families on managing illnesses or injuries and promote healthy lifestyle choices.
• Coordinate with healthcare providers to ensure continuity of care.
• Maintain accurate and up-to-date patient records and documentation.
• Report patient status and any significant changes to the supervising RN or physician.
• Document all care provided and any patient interactions in compliance with regulatory and agency standards.
• Provide guidance and education to patients and their families regarding medication management, treatment plans, and self-care techniques.
• Offer emotional support and resources to help patients and families cope with health issues.
• Follow infection control protocols and ensure a safe environment for patients and staff.
• Adhere to agency policies, procedures, and regulatory requirements.
• Maintain confidentiality of patient information in accordance with HIPAA regulations.

Requirement:

• BLS
• LPN state or compact license
• Covid Card
• Flu Vaccine
