Responsibilities
- Accountable for overall treatment planning efforts, care conferences with families and providers, family and provider care updates, psychosocial needs and completion.
- Coordinate and facilitate the development of a discharge plan of care for the patient population which includes accountability for the complex discharge process.
- Ensure the experience of care for the patient and family is exceptional.
Qualifications
Education:Bachelor's Social Work (BSW) required; Masters of Social Work (MSW), Counseling, Psychology, or Psy. D preferred.Experience: Two-Three years of social work experience in inpatient behavior health setting or acute psychiatric settingLicensure: Current Indiana Social Worker license is highly preferredClinical Social Worker Licensure preferred. CPR and CPI certification obtained during orientation.#INDEEDMED
