The Intake Specialist role provides frontline intake services to members joining New Freedom’s Transitional Program. The Intake Specialist is one of the initial points of contact to ensure members have a positive and successful entry into the Transitional program. This position is accountable for reviewing eligibility and program expectations prior to members meeting with their assigned clinical staff/team. This position requires a thorough understanding of the company’s philosophy of treatment, admission processes, and program criteria. A basic knowledge of substance abuse and dual-diagnosis is also required. The Intake Specialist is responsible for ensuring that clinical and administrative intake services are provided for members, in partnership with the clinical team and other cross-functional departments.
Duties and Responsibilities:
- Completes initial phone screens
- Initiates intake packet/paperwork and other necessary consent/admission forms
- Completes the intake assessment, effectively and correctly identifying and documenting appropriate diagnoses
- Completes the individualized treatment plan, based on the treatment goals identified through the completed assessment
- Explains the program in a thorough manner, ensuring that the member fully comprehends the informed consent provided and acknowledged through their signature, including but not limited to the following:
- State specific requirements, program policies, and the need to report any suspected child abuse and neglect forms of abuse
- Verification of the member’s involvement in collaborative treatment options, to include their goals for treatment
- Importance of treatment compliance
- Resources for comorbidities and/or dual-diagnosis, if needed
- Members' rights and responsibilities
- All program options available to member and the member’s rights and responsibilities in the program
- Grievance and appeals procedures
- Ways in which input is given regarding the quality of care, achievement of outcomes, and satisfaction of the person served
- The organization's services and activities; expectations; hours of operations; access to after-hour services; code of ethics; and confidentiality policy
- Program's policies regarding use of tobacco products, illicit or licit drugs brought into the program, and weapons brought into the program
- Identify the person responsible for coordinating services for the member
- Provide a member handbook explaining the program, expectations, rules, rights, and explanation of behaviors or attitudes that may lead to the loss of rights or privileges
- Education regarding advance directives
- Identify the purpose of the assessment and the assessment process
- Voluntary participation in the program
- Compliance with federal confidentiality regulations
- Orientation on the first day of treatment, if possible, but no less than within seven (7) working days of admission to treatment
- Completes ITIS Orientation classes at the Dunlap, Peoria, Deer Valley, and/or any new facilities as assigned and as required
- Completes hand-off to the clinical and cross-functional member support departments once all intake tasks are successfully completed for the member
- Effectively meets any/all outlined Key Performance Indicators (KPIs), ensuring the highest quality and level of member care and engagement is provided
- Receives and actively engages in clinical supervision and oversight each week/month based on BHPP/BHT designation requirements, including completion of all continuing education requirements
- In addition, all other duties as assigned
