Reports To: Life Skills Program Manager Approved – 2022.10.19 LS-04
Summary/Objective
Support individuals with intellectual and developmental disabilities to reach their individual goals by teaching skills in a nurturing environment so they can have full access to their community. Responsibilities include, but are not limited, providing personal assistance, administering medication, instructing in activities of daily living and tasks completion, as well as transportation to and from community resources and events. Developing schedules and communicating with DSP’s. Experiences working with individuals with disabilities is helpful, but not required.
Essential Functions/Job Duties

- Use knowledge, skills (competencies), and values (Code of Ethics) to guide their work and provide quality services to assist people who need support to lead self-directed lives and participate fully in their communities, while recognizing the unique gifts, preferences and needs of each person supported.
- Maintain compliance to governing authorities including the Bureau of Quality Improvement in Services, Rule IAC 460, Department of Labor, and the agency Quality Management System.
- Ability to communicate well and interact with all types of personalities.
- Assists the person served to meet his or her physical (e.g., health, grooming, toileting, eating), and personal management needs (e.g., human development, human sexuality), household management needs (e.g., meal prep, laundry, cleaning, decorating), and with transportation needs by teaching skills, providing supports, and building on individual strengths and capabilities.
- Partner with the persons served in making informed decisions and everyday choices, while respecting the privacy of the people they serve.
- Empower people they support to have meaningful relationships with friends, their families, and promote integration and inclusion in our communities while advocating with them for their valued role in society.
- Understand that their role includes being a mentor and a facilitator and that quality services occur at the point of interaction with the people that they support, providing them choices, engaging, and encouraging them to participate in activities.
- Provide opportunities for the person served to develop, maintain, or practice skills outlined in the PC-ISP and record the efforts made by the person served.
- Provides and accepts co-worker support, participating in supportive supervision, performance evaluation, and contributing to improving the quality of service.
- The knowledge and ability to use and have access to a computer, cellphone, or tablet, as well as the willingness to receive additional training to use such devices to assist in providing service to the persons served.
- Continue to learn and develop their professional abilities through ongoing in-service and self-directed education and training.
Facilitation of Services
The Team Leader is knowledgeable about a range of engaging and participatory planning techniques and is skilled in implementing plans and following schedules in a collaborative and expeditious manner.
Organizational Participation
The Team Leader is familiar with the mission and practices of the support organization and participates in the life of the organization.
Supervisory Responsibility
This position has no supervisory responsibility.
Work Environment
This position routinely is performed within the New Horizons facilities and scheduled community activities.
Physical Demands
Demonstrate physical agility to walk, run, bend, squat, grasp, lift fifty pounds and perform other moves in providing assistance.
Equipment, Machines, Tools and Vehicles Used
Computers, cellphones, and/or tablets, along with various virtual platforms.
Position Type/Expected Hours of Work
Day shift/ 8 hour weekdays
Required Education and Experience
- High school diploma or equivalent
- Must possess or be able to complete requirements for First Aid/CPR certification
Additional Eligibility Qualifications
- Eighteen (18) Years of Age or Older
- Ability to communicate adequately to complete required forms and reports of services provided; follow oral and written instructions; and communicate with the person served in the individual's mode of communication.
- Complete training and demonstrate continuous competency.
- Have and maintain Valid State-Issued Driver’s License
- Have and maintain state minimum Vehicle Insurance Coverage if transporting a person served.
- Have and maintain an acceptable Bureau of Motor Vehicles driving record.
- Have a valid social security card.
- Must be able to pass all applicable background checks.
- Must be able to safely utilize transport equipment such as wheelchairs and walkers.
- Must pass a drug test and TB screening.
