Case Manager
NewBridge is a non-profit organization that helps provide older adults a bridge to successful aging. Our agency provides a wide variety of services and programs primarily for low-income individuals, aged 60 and older. We are seeking someone who is passionate about seeing older adults thrive and has experience and relatability in racially diverse settings, especially with Black older adults. NewBridge intentionally values and promotes diversity, equity, inclusion and anti-racism in serving older adults. The case manager will work with low income older adults in Madison and Monona, many of whom will be Black.
Qualifications:
Case management is a method used to help individuals to stay safe and well, both physically and emotionally. An effective case manager helps low-income older adults connect to resources, services and opportunities to ensure they can remain independent. Our case managers work to build trust and develop a professional relationship with each older adult. They are successful because they are good communicators, provide knowledge of local resources and are strong advocates for those they serve.
Responsibilities:
● Complete comprehensive needs assessment with client
● Conduct regular visits with assigned clients; often in-person and at the client’s home
● Develop an individual service plan with the client based on their needs
● Coordinate and arrange for services based on available resources
● Provide follow-up, support and advocacy on behalf of the client to access needed services
● Create and maintain confidential client case notes
● Answer calls on our intake phone line
Education and/or Experience:
● Associate, Bachelor’s or Master’s Degree in Human Services or related field OR equivalent life experiences
● Computer skills, specifically with Google Workspace or a willingness to learn
● Ability to embrace diverse perspectives and understand the relationship between race and life experiences
Requirements:
● Driver’s license with access to a reliable personal vehicle
● Automobile insurance
● Personal cell phone
● Criminal/caregiver screening
Schedule:
● 40 hours per week
● No weekends, holidays or on-call
Salary & Benefits:
$45,760 - $49,920 annually
