Location: Great Bend, Kansas

Salary: $25.51 - $41.06/hour

Shift: Full-Time, Days | Monday-Friday, 8-hour shifts

Job Description

We are seeking a compassionate and dedicated Home Health Social Worker (LMSW) to provide support to adult and pediatric patients within a home health setting. In this role, you will work primarily with adult patients, but we welcome candidates who are willing to work with all ages. You will split your time between clinic visits (60%) and hospice services (40%). As part of this role, you will occasionally participate in support groups that meet on Tuesday evenings. This position does not require on-call work except for emergency situations.

Key Responsibilities

Provide emotional support, resource guidance, and counseling services to patients and their families in a home health and hospice setting.

Conduct assessments to determine the psychosocial needs of patients and provide care plans accordingly.

Facilitate community support groups as needed, typically on Tuesday evenings.

Coordinate with healthcare teams, including nursing staff and physicians, to ensure comprehensive care for patients.

Travel between patient homes, hospitals, and clinics within a 30-mile radius, with mileage reimbursement provided.

Qualifications

Required:

Masters degree in Social Work (LMSW).

Current social worker licensure (LMSW) or LSCSW with the Kansas State Board of Social Work.

Basic Life Support (BLS) certification.

Preferred

Licensed Specialist Clinical Social Worker (LSCSW).
