SUMMARY:
Under limited direction plans, coordinates, manages and directs all operations of the assigned SOURCE Care Management location. Implements guidelines of SOURCE Care Management; leads data analysis for assigned location and communicates effectively on operations to the Regional Director, as a primary job function.

- Thorough knowledge and enforcement of current regulations, as well as SCM Operational guidelines and best practices as related to the operations of the SOURCE Program and SCM.
- Thorough knowledge, adherence and enforcement of SCM personnel guidelines.
- Maintains constructive working relationships by communicating and interacting effectively with supervisors, organizational leadership, peers and individuals inside and outside the Organization, in a positive, professional and respectful manner.

- Bachelor’s degree in Social Work or related field with minimum of 2 years of experience in the health and human services field with at least 2 years in management or supervisory position OR
- Valid RN license with a minimum of 2 years experience in health and human services field with at least 2 years in management or supervisory position OR
- Valid LPN license with a minimum of 3 years experience in health and human services field with at least 3 years in management or supervisory position.
- Experience in social work, home and community based services, healthcare or geriatrics preferred.
- Valid Driver’s License.
- Reliable transportation.
