What are the responsibilities and job description for the Clinical Specialist - Respiratory Navigator position at Northeast Alabama Regional Medical Center?
Reporting to the Respiratory Services Supervisor and the Director of Respiratory Services, this position is responsible for ensuring that all educational needs of the department are met. Locate, Identify and evaluate patients with following primary diagnoses: COPD, COPD exacerbation, bronchiectasis with exacerbation, respiratory failure, dyspnea, pneumonia, asthmatic bronchitis, status asthmatics, and asthma exacerbation who would benefit from respiratory navigation guidance. Provides materials appropriate for patient#s educational level/special needs to patients meeting the criteria for inclusion, including disease overview, triggers, medication schedule and side effects, correct technique of MDI#s and home nebulizer equipment, self-management skills, action plans and smoking cessation. Locate, identify, interview and educate (at risk) patients of COPD, Asthma and OSA (Obstructive Sleep Apnea). Work closely with Respiratory and Nursing staff to identify patients in need of a coordinated approach to care facilitating transitioning forward to discharge. Work closely with Physicians, Pulmonologist and Hospitalist as a point of contact for coordination of care and communications while keeping them informed on progress. Work closely with Respiratory Therapist and Clinical Specialist in coordinating care. Work closely with Transition of Care to provide evaluation of COPD, Asthma and pneumonia patients within their target groups with focus on admissions and re-admissions. Work closely with Social Services with substantial coordination to ensure a smooth transition to a post hospital environment of which a therapeutic level of care can be maintained to provide optimal care for pulmonary patients. Work closely with DME companies to ensure correct implementation of CPAP, BIPAP, oxygen and other related respiratory equipment. Associates Degree required, Bachelor#s Degree preferred. RRT preferred Current license in good standing with the Alabama State Board of Respiratory Therapy CRT required from the National Board for Respiratory Care ACLS # BLS required
