Title: Program Case Manager
Job Summary: Provides supervision, opportunities, and skill development to participants served NeArc’s community-based day and work supports program.
Major Responsibilities:
- Evaluates and determines program participants’ interests and develops individual skills.
- Provides oversight to assigned caseload by attending scheduled meetings, maintaining regular and professional contact with the participant’s team, completing all required documentation, and ensuring the participant’s progress in program as determined by his or her goals.
- Creates individual goals to reflect the participant’s vision regarding skill development.
- Completes all necessary assessments and required paperwork for the participant’s annual support meeting in HCSIS within the required submission timeline.
- Develops and facilitates meaningful activities for program participants within the program and community.
- Maintains all required documentation within required timeframes to include:
- Monthly/Quarterly Progress Notes/6-month summaries in HCSIS
- Interest Inventories
- Assessments
- Confidential Record updates
- Any other required paperwork as assigned by supervisor.
- Serves as a role model and assists in developing interactive skills.
- Assists in obtaining appropriate volunteer placements for participant served and providing ongoing coaching support in these placements to ensure continued success.
- Attends and completes all required trainings assigned by the supervisor.
- Meets the physical and personal care needs of participants supported by the Program.
- Other duties and special projects as assigned.
Education and Experience: High School Diploma or equivalent required. 1 year experience working with intellectually and/or developmentally disabled adults or other related experiences preferred.
Physical Demands: Must be able to stand/walk up to one hour uninterrupted.
