Summary

The Care Connector will plan, develop, coordinate and execute various direct service programs. The Care Connector will play a critical role in helping the agency achieve its vision to provide high quality programs.

Under the direction of the Direct Programs Manager, the Care Connector will be responsible for a variety of significant activities including program development, program implementation and expansion, and program evaluation services.

The primary programs this Care Connector position will work within are ElderSource’s social engagement programs, which will include overseeing a volunteer delivered telephone reassurance program. This Care Connector position may be assigned secondary special project programs that address social isolation and loneliness in older adults, such as programs delivered through tablets or websites. This position will be cross-trained in programs delivered by other Care Connectors.

  • Essential Duties and Responsibilities include the following. Other duties may be assigned.
  • General Program Development and Implementation:
    • Coordinate all aspects of assigned program(s)
    • Develop partnerships to implement program(s) that will maximize revenue and produce quality outcomes
    • Contribute to programmatic marketing strategy development and execution
    • Represent the organization at a variety of functions and public/private meetings
    • Develop, review, and update policies and procedures and handbooks for assigned program(s)
    • Develop and monitor quality assurance indicators, including satisfaction surveys
    • Ensure ongoing programmatic excellence through evaluation and, when applicable, program usage/participant reports.
    • Recommend timelines and resources needed to achieve assigned program goals
    • Work collaboratively with ElderSource inter and intra-department staff to ensure the success of each program
    • Conduct telephonic, virtual, or on-site outreach to recruit participants as needed for assigned program(s)
    • Ensure quality programming through proper implementation and monitoring program reports
    • Complete assigned program(s)invoicing for proper payment
    • Complete all trainings and certifications required to execute assigned program(s)
  • Direct Service Program Implementation
    • Develop and maintain a record or database of participants, as appropriate, in assigned program(s)
    • Determine nature of participants situation through structured conversations, assessing medical, psychological, emotional and social information and, when needed, making on-site visits
    • Utilize participant onboarding results to identify potential interagency referrals, and, when required by program, for creation of individual care plan or volunteer/intern matching
    • Obtain assistance for participants by pre-screening community resources, when possible, referring participants to community resources, and establishing rapport with other agencies
    • Foster participant action or adjustment by interpreting attitudes and patterns of behavior, explaining and suggesting new options as needed
    • Maintain records by following assigned program(s)’ policies and procedures for documenting participant situations, actions, and contacts; including any APS reports, Wellness Checks, or 911 calls.
    • Monitor discussed actions through periodic follow-up, when needed.
    • Maintain operations by following policies and procedures, participating in quality reviews and using review results to inform direction of assigned program.
    • Monitor participant usage of tablet program.
    • Provide assistance and follow-up of tablet users, as needed.
    • Comply with federal, state and local legal requirements by studying existing and new legislation, enforcing adherence to requirements and advising management on needed actions.
    • Maintain participant confidence and protect operations by keeping information confidential.
    • Collaborate with internal and external stakeholders to ensure quality outcomes for participants.
    • Implement program activities according to best practices and standards.

II. Supervisory Responsibilities

This job has no staff supervisory responsibilities. If assigned program utilizes interns, staff will carry out intern supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities may include interviewing, training,; planning, assigning, and directing work; appraising performance; rewarding and disciplining interns; addressing complaints and resolving problems.

III. Education and/or Experience

Bachelor’s Degree and six months of related experience, or equivalent combination of education and experience, required. Preferred education to be in gerontology, public administration, veterans issues, health and human services, and/or social services. Must be proficient in Microsoft Office and have excellent communication, training, and presentation skills.

Pre-employment background check and drug screen will be conducted on top candidate. It is mandatory that a candidate be able to pass a Level II criminal background check through the Department of Elder Affairs prior to starting work.

Salary: $32,718.40 to $35,990.24/year
