Job Title: COMMUNITY LIVING TRAINER
Reports to: House Manager, Director of Operations
General Description
The primary goal of this position is to maintain member safety as well as stimulate and encourage the member to reach their full potential. This includes consistent supervision, achieving daily goals, and implementing a variety of activities for member growth and entertainment. This may also require the provider to transport members as well. Providers attend monthly educational meetings that include learning changes and techniques within the field, modifications and approaches with members, as well as interpersonal skills to provide a positive working environment. Providers also have a variety of shift responsibilities they complete on a weekly or daily basis.

· Ability to deal courteously and tactfully with other employees, superiors, members’ families, and the public.
· Ability to maintain patience and perseverance with members.
Essential Job Functions
· Actively supervise and protect the safety of members and other providers.
· Assure safety of members and providers at all times (follow emergency drill procedures and individuals’ positive behavior support plans).
· Be aware of surroundings and members’ needs/behavior at all times. All providers must stay observant of individuals.
· Achieve daily member goals and document all activities that pertain to the shift working correctly. Run and score the goals correctly.
· Document all required fields in the community living schedules and MAR’s, as well as any needed paperwork regarding individuals being served (Critical and internal incident reports, personal needs list, money management, etc.). All staff must sign, print, initial and date on all required documents billed to the state.
· Community living providers are assigned specific duties by the House Manager that need to be completed. These may include: information needed to facilitate continuation of care for individuals being served at the end of shifts, documentation of incidents which occurred on shift, and remaining in communication with other staff and supervisors.
· Create and implement activities appropriate to members’ abilities and ensure safety during such.
· Ensure that each individual served is receiving the highest quality of care and needs are met at all times.
· Build staff/individual working relationships with consistent interaction with individuals served.
· Positivity, encouragement, and promotion of independence.
Meal Preparation/Groceries – Community living trainers assist in preparing meals for two to four individuals, taking into account the likes, dislikes, and dietary needs of individuals being served.
Trainers may be asked to prepare menus and shopping lists, following the monthly grocery budget and shop for the groceries as approved by House Managers/Program Director.
Keys/Security - Any keys giving access to restricted or protected items or locations MUST be kept on your person at all times. All staff is to lock up keys that pertain to any medications in a locked box with a specific combination that is to be memorized. This information is confidential and may result in employee disciplinary action or may result in suspension of duties if it is shared.
Medication cabinets and doors are to be locked at all times. Please keep keys with group home shift employees at all times. If keys cannot be on staff’s person, then all keys must be kept in a locked area or the required lock box at all times.
Transportation - Follow transportation guidelines and ensure safety. Community living providers may be required to transport individuals to needed activities and outings. Staff providing transportation must have a valid Wyoming driver’s license and insurance.
Documentation - Fill out appropriate paperwork. All paperwork must be completed in black or blue pen, making sure to initial all appropriate places.
Paperwork must be complete and on time, by Monday morning each week. All documentation should be properly completed before the last shift of employee’s work week.
Cleaning - Residence should be kept clean, neat, and tidy at all times, following all shift responsibility guidelines.
Professional appearance – Community living trainers should maintain an appropriate, professional appearance and behavior, as specified in the employee handbook, at all times.
Once community living trainers are fully trained and cleared to work on their own, trainers are encouraged to be confident, independent, and honest in their position.
