Provides drug information and assists the interprofessional care team in providing optimal pharmaceutical care for patients. Completes required learning experiences and select elective learning experiences, assigned residency projects, and educational competencies, goals and objectives as required by the individual program and as defined by the American Society of Health-System Pharmacists. Acts as a pharmacist when licensed in New York State
Job Responsibility
1.Collaborates with the health care team, and provides comprehensive medication management to patients following a consistent patient care process.
2.Ensures continuity of care during patient transitions between care settings.
3.Manages and facilitates delivery of medications to support safe and effective drug therapy for patients in the advanced practice area.
4.Manages formulary and medication-use processes for patients, as applicable to the organization.
5.Conducts a quality improvement or research project.
6.Participates in activities designed to advance the profession of pharmacy.
7.Provides medication and practice-related education to patients, caregivers, health care professionals, students, and the public
8.Engages in precepting and teaching students, pharmacy technicians, or fellow health care professionals.
9.Participates in assuring pharmacy compliance with internal and external compliance requirements, including legal, regulatory, safety, and accreditation requirements, when applicable.
10.PGY2 Residents perform all responsibilities according to their advanced area of pharmacy practice.
11.Performs related duties as required, outlined by individual program manuals.

Job Qualification
1.PGY1
Doctorate of Pharmacy (PharmD) from an Accreditation Council of Pharmacy Education (ACPE)-accredited school of pharmacy, required.
Eligible for pharmacist licensure in New York State.
2.PGY2
Doctorate of Pharmacy (PharmD) from an Accreditation Council of Pharmacy Education (ACPE)-accredited school of pharmacy, required.
Eligible for pharmacist licensure in New York State.
Completion of a ASHP-accredited Post-Graduate Year 1 (PGY1) residency program, required.
