Job Description
PURPOSE OF POSITION:
The Patient Care Associate is an auxiliary worker who under the direct supervision of the Registered Nurse, the Licensed Practical Nurse or other assigned professional staff members who perform certain functions for the support of Nursing Service.
RESPONSIBILITIES:
- Performs physical care of assigned patients, obtain vital signs, weight, height, blood pressure, oral intake and output as assigned and notify nurse of findings.
- Prepares patient for examination and /or special procedures and accompany patient if required.
- Observes patients' condition, mental state and social behavior and reports to Nurse-in-Charge.
- Prepares patients for. meal and assists with feedings as needed.
- Distributes nutritional supplements to patients and documents on I & O sheet.
- Assists and accompanies patients during the admission, discharge or transfer process, assuring comfort and safety.
- Assists patients with activities of daily living.
- Demonstrates competency in assisting with code management.
- Turns and positions patients as instructed by RN or designee.
- Participate in pain management and informs nurse of finding in timely manner.
- Monitors patient on suicidal precaution and notify the nurse of observations and patient's behavior.
- Monitors patient on fall precaution and complete Flowsheet as per policy.
- Collects routine specimens, such as urine, stool, sputum, etc.
- Performs weights as requested and documents appropriately.
- Provides Post-Mortem Care.
- Reports patient care, improvements and/or problems to Head Nurse/or designee in a timely manner.
- Makes rounds willingly and frequently and anticipates patients' needs.
- Responds to patient call lights promptly.
- Records on designated forms patients' personal care, vital signs etc. or other observation.
- Carries out established Infection Control, protocol and rv1RI Safety.
- Notifies Head Nurse/Charge Nurse of insufficient supplies and malfunctioning equipment.
- Keeps patient’s immediate environment free of clutter.
- Follows established procedure in securing patients clothing and personal property, e.g. prosthesis, dentures, eyeglasses, hearing aids, wheelchair, etc.
- Conforms to policies governing patients' abuse, patient rights and confidentiality of information.
- Participates in Performance Improvement (PI) Activities.
- Demonstrates cultural sensitivity to diversity in work force.
- Attends In-Service offered by staff development in order to comply with regulatory mandates and foster self-development.
- Conforms to policy governing uniform codes, attendance and punctuality. Performs all other duties as delegated by Nursing Administration.
- Verbalizes awareness of team building.
- Demonstrates communication skills including Limited English Proficiency (LEP) and the use of the Cyracom Phone.
- Communicates to the Head Nurse of designee problems or issues affecting patient care.
- Assists in preparing patients for surgery such as bowel preps and bathing.
- Post-operative care facilitating early ambulation.
- Assists in the care of patients with but not limited to COPD asthmas OM, HTN, HIV, pneumonia, CHF, Head Trauma, Cholecystectomy.
- Ensures a safe and secure environment free of hazards to reduce the risk of injury to all patients, staff and visitors.
- Responds to and differentiates clinical alarm systems.
- Obtains EKG on patient with MD order for urgent or stat cases.
- Obtains/draws blood speciment on patient with MD order for urgent or stat cases.
- Follows correct patient ID procedure and labeling of specimen in front of the patient.
- Prepares admission/transfer/discharge as per protocol.
Duties & Responsibilities
1. Checks vital signs, weighs, and measures patients, obtains specimens and records findings on patient’s chart.
2. Instructs patients on how to obtain specimens for laboratory testing and performs basic specimen screening testing through dipstick and hemocult.
3. Prepares collected specimens for pick-up by labeling and preparing required form(s).
4. Performs vision and hearing testing and simple urinalysis.
5. Administers emergency first aid, including basic life support procedures.
6. Performs simple first aid such as dressing changes, bandaging and cleansing of wounds, irrigation and cleansing of ears.
7. Prepares patient for examination and assists medical provider during examination by providing necessary supplies/equipment.
8. Prepares examination room including cleaning and disinfecting of instruments and equipment.
9. Informs patients on proper preparation for various medical testing.
10. Interviews patient to obtain routine personal and medical information and appropriate health history.
11. Maintains, completes, processes and files patients’ charts, appropriate forms and documents.
12. Reports observations of patient to clinical staff caring for patient to ensure professional assessment.
13. Answers routine inquiries of patients in person or via telephone and makes appropriate referrals.
14. Provides information to patients and their families regarding health care services and the achievement of healthy, safe living.
15. Maintains inventory of the unit’s equipment stock and supplies.
16. Works effectively as a team member in the delivery of care and assists in the orientation of new employees.
17. Directs and escorts patients to other departments and/or health care settings, as needed.
18. Coordinates efforts of various clinic and health care setting departments in scheduling and re-booking appointments through manual and/or automated systems and enters pertinent patient information into the Electronic Medical Record systems.
19. Obtains blood specimens for laboratory testing by performing venipuncture and/or fingerstick testing.
20. Operates electrocardiograph equipment to record electrical wave tracing of the electrical currents of the heart muscle.
Minimum Qualifications
1. High school diploma or its educational equivalent, approved by a State’s Department of Education or a recognized accrediting organization; and
2. Successful completion of a NYS Department of Health prescribed training program for Phlebotomy, satisfactory to the Laboratory Director; and
3. Completion of a health care setting-based training program in the performance of electrocardiographs and basic life support procedures including certification in Cardio-Pulmonary Resuscitation.
Special Note:
In addition to meeting the qualifications above, assignment to a long-term care facility requires a New York State Nurse’s Aide Certificate.
