POSITION DESCRIPTION
Job Title: Intake Coordinator; Reports to: Clinical Director
- The mission of the O’Brien House Intake Care Coordinator within the Treatment department is to process, schedule and ensure all Clinical needs of clients are complete, such as: screening, admitting, life skills, referrals, educational assessments, treatment planning, employment, discharge, etc.
As the SRO Case Manager this person serves as a liaison between the SRO tenants managed by O’Brien House and the Louisiana Balance of State Continuum of Care/Louisiana Housing Corporation, whom provides federal grant money through HUD for case management.
Education/Experience Minimum Requirements: An Intake Care Coordinator must meet the following qualifications:
- Master’s degree in social services discipline; or bachelor’s degree in a social services discipline with a least one-year relevant experience; or High School diploma with at least (4) years relevant experience.
- Intake Coordinator will work with the Clinical Director prior to admission to ensure the consumer is properly linked to the appropriate levels of care by pre-screening applications. An intake/care coordinator should not be assigned as the client primary counselor.
Job Duties:
1) Work as a liaison between O’Brien House and the community at large for client referrals. The role includes the following tasks: a. Maintain open and professional communication with agencies, individuals and families interested in client placement at the facility b. Ensure that all screening packet information needed for potential intake is up to date c. Communicate with the IT/Website manager to ensure that all forms are up to date on the website d. Provides community resource outreach and networking
2) Once a client referral packet is sent into the facility, intake coordinator will enter the information (name, date received, facility received from, and potential admit date) on the Intake Log. Once the referral is added to the intake log it will be passed to a designated staff for clinical review and approval/denial by the Clinical Director or designee. a. The Intake Referral Log should be emailed to the Clinical Director at least once a week.
3) If a client is approved, they are then placed on the waiting list. If a client is approved, the referring provider is contacted with a recommendation for alternate placement. a. The Waiting List should be sent to the Clinical Director and designee assigned to complete the daily census at least 3 times weekly for updating and accuracy.
4) If the decision is made to approve a client, the referring agency is contacted, and a client is scheduled if beds are available. The intake coordinator is then responsible to ensure that the referring agency is made aware that a 30-day supply of any prescribed medication is provided to the client prior to intake at O’Brien House by having a pre-filled prescription or a written prescription for a 30-day supply of medications. The intake coordinator is also responsible for informing the referring provider with information regarding approved items and necessary required documentation for intake. a. The Intake Coordinator should communicate with referring agencies on a regular basis update to policies and procedures relevant to the needs of the clients.
5) Creation of Residential Client Charts and ensuring that all forms are relevant, accurate and up to date. a. Inform the Clinical Director or designee when new forms or update to forms are required.
6) Conduct Intake work (1 and 1 ½ hours per client) by reviewing, signing and obtaining client signatures on all of the intake documents.
7) Works with Care Coordinator to maintain and update the Person-Centered Training and Groups
8) Assist with new intake processing when the IOP Intake Coordinator is not available.
9) Works with Clinical Director, Counselors, and Care Coordinators in the following areas if needed: a) Ensures that assigned clients keep their regularly scheduled appointments and attend mandatory meetings as it relates to obtaining state identification and social security card. b) Plans discharge referrals and release of information as deemed appropriate. c) Bring to the attention of the counselors and/or immediate supervisor any problems concerning clients. d) Ensures recovery support services assigned are appropriate for the identified level of care and stage of recovery. e) Assist Case Manager’s as needed with client’s needs (medical, dental, mental health, housing, etc.)
10) Assist Case Managers with making monthly follow-up contact with clients when they leave the program when appropriate.
11) Meets weekly with the Clinical Director and Treatment Team for Client Staffing to discuss individual evaluations and planning for clients in the program.
12) In addition to the standard chart forms, ensure that client information is entered in KIPU and Weekly Client Service Logs in a timely and properly manner.
13) Available for Saturday and Holiday Rotation Schedule and for weekend meetings, staffing and trainings as scheduled.
14) The intake coordinator will maintain all professional credentials by attending Continuing Education as required by state mandated License, if applicable.
15) Follow the Rules and Regulations as stated in Employee Handbook and PPM.
16) Adhere to the Codes of Ethics.
17) Performs all other duties as assigned by the Clinical Director and Executive Director.
