Description

JOB TITLE: Clinical Director

JOB SUMMARY: This position requires a Registered Nurse who will be responsible for coordinating all nursing units for all shifts in directing the care of the resident. Makes recommendations for improvement to the Director of Nursing. Works with the nurses for the coordination of patient care. Is responsible for making rounds and assisting the Medical Director and/or patients’ physicians.

ACCOUNTABILITY: Director of Nursing

EQUIPMENT USED: Various medical equipment, instruments, and machines including a computer and/or typewriter.

ENVIRONMENT: 100% inside.

DUTIES AND RESPONSIBILITIES:

Except as specifically noted, the following functions are considered essential to this position.

NOTE: Certain functions may vary by facility.

1. Ensures that at time each resident is admitted, obtains physician orders for immediate care; physician orders received and signed as required. Faxes order to pharmacy. orders regarding CPR and DNR are obtained by the Admissions Coordinator. Assists unit nurses with physical assessment on each admission to obtain base line data and document.

2. Ensures that each resident receives the necessary care, appropriate treatments, assistive devises and other services to attain or maintain the highest practicable physical, mental and psychosocial well-being in accordance with the plan of care.

3. Ensures resident maintains highest practical physical well being.

4. Works with Dietary department to ensure that resident maintains acceptable parameters of nutritional status as laboratory values, body weight and protein levels, unless clinical condition demonstrates that this is not possible; therapeutic diet initiated; provides fluid intake sufficient to maintain proper hydration.

5. Ensures that special needs of residents are met in such areas as: injections, parenteral fluids; ostomy care; tracheostomy care, suctioning and respiratory care; foot care; prosthetics.

6. Monitors drug regimen with consultation of pharmacist and physician to avoid unnecessary drugs; avoid or reduce antipsychotic drugs; assure that medication error rates are monitored and kept at a minimum.

7. Communicates with the physician, director of nursing, administrator and other health professionals as appropriate to provide necessary services to residents.

8. Makes rounds with Medical Director and other staff physicians, giving report of resident’s clinical status and initiating orders as written.

9. Makes rounds of all units at beginning and end of shift; gives 24 hour report to on-coming Supervisor/Charge Nurse.

10. Responsible to know the facility policies and procedures and able to communicate and interpret to staff.

11. May be required to schedule and replace staff sufficient to meet needs of patients; minimum staff based on state and federal requirements.

12. Assists with the orientation of new nurses; responsible to supervise all professional and non-professional nursing staff.

13. Attends in-service programs; professional seminars and workshops to promote both individual and professional growth.

14. Attends facility meetings as requested and/or required.

15. Assists in keeping residents’ records medical records current.

16. Does all things necessary for the patient’s health, safety and welfare.

17. Must consistently demonstrate a positive attitude and behaviors, while contributing to the overall success of the Company.

18. All other duties as assigned by supervisor.

Qualifications


Physical Activity Requirements:

PRIMARY PHYSICAL REQUIREMENTS:

LIFT up to 10 lbs: Frequently required when lifting charts weighing approximately 5 to 10 lbs.

LIFT 11 to 25 lbs: Occasionally.

LIFT 26 to 50 lbs: Occasionally.

LIFT over 50 lbs: Occasionally.

CARRY up to 10 lbs: Occasionally may be required to carry medical charts from one location in facility to another (50 to 100 feet). If carrying more than one chart, can implement the use of a cart.

CARRY 11 to 25 lbs: Not required.

CARRY 26 to 50 lbs: Not required.

CARRY over 50 lbs: Not required.

REACH above shoulder height: Occasionally may be required to reach for charts or supplies on upper shelves.

REACH below shoulder height: Constantly required while documenting charts, drawing blood, and providing care to residents.

PUSH/PULL: Occasionally may be required to push/pull medication chart, desk drawers, or resident wheelchairs.

HAND MANIPULATION: 3

Grasping: Constantly during the course of the day while documenting medical files and other records.

Handling: Frequently handling paperwork and occasionally various diagnostic tools such as thermometer, blood pressure gauge, etc.

Torquing: Required for opening some medications and tube feedings.

Fingering: Occasionally may be required for using typewriter, computer keyboard and telephone.

Controls & Equipment: Facility may implement the use of a computer.

OTHER PHYSICAL CONSIDERATIONS:

Twisting: Not required.

Bending: Occasionally may occur when drawing blood or taking vital signs.

Crawling: Not required.

Squatting: Not required.

Kneeling: Not required.

Crouching: Not required.

Climbing: Not required.

Balancing: Not required.

DURING AND 8 HOUR DAY, EMPLOYEE IS REQUIRED TO:

Sit 2 Consecutive Hours 4 Total Hours

Walk 2 Consecutive Hours 3 Total Hours

 

WORK SURFACE:

Employee may be required to stand on carpeting, linoleum, or be seated at a standard desk at the nurses station or office using an office chair.

 

Cognitive and Sensory Requirements:

Talking: Necessary for communicating with others.

Hearing: Necessary for taking instructions from physicians, supervisor, and administration.

Sight: Necessary for doing job effectively and correctly.

Tasting & Smelling: Required for accurate maintenance and detection of wounds, urinary tract problems, etc.

 

Specific Vocational Preparation Requirement:

Over 2 years up to and including 4 years.

 

Licensure/Certification Requirements:

Current license of the State in which the nursing services are provided.

 

Other Training, Skills, and Experience Requirements:

Needs to have worked 2 years in a long term care facility. 4

Tasks and procedures performed by employee involve risks classified by CDC as:

Category I (Direct contact with blood or other bodily fluids to which universal precautions apply).

Category II (Activity performed without blood exposure but exposure may occur in emergency).

Category III (Task/activity does not entail predictable or unpredictable exposure to blood).

This position typically does not involve Category I exposure risk; however, if employee is trained in first aid, some emergency procedures may entail Category I exposure risks.

 

Other Considerations and Requirements:

Employee must be able to tolerate prolonged sitting as required when documenting information in medical charts for residents. Must be able to direct and work in a high-paced nursing facility.
