What are the responsibilities and job description for the Case Manager/Home Manager position at Open Doors, Inc.?

Case Manager/Home Manager position in Title XIX I/DD Waiver services. CM links people with developmental or intellectual disabilities with services, acts as an advocate, facilitates meetings, interacts with billing and pre-authorization entities and ensures that services are being provided per the IPP. This position requires on call. Home manager oversees basic needs within the home of Waiver members who reside in 24 hour sites.

Job Type: Full-time

Pay: $15.00 - $18.00 per hour

Expected hours: 40 per week
