Resource Coordinator of Community Services (RCCS)


POSITION SUMMARY: The Resource Coordinator of Community Services (RCCS) will be working closely with assigned CCSs to ensure quality of services provided by the CCSs to OHC Inc. clients. There will be 1 RCCS for 10 CCSs. The RCCSs will provide oversight of case management services rendered as well as consultation and relevant training to CCSs.


QUALIFICATIONS:
  • Bachelor’s degree with:
    • 3 years experience in coordinating services for people with developmental disabilities or 5 years of experience working in the Human Services field.
OR
Master’s degree with
  • 1 year experience in coordinating services for people with developmental disabilities or 3 years of experience working in the Human Services field
  • Have demonstrated skills and working knowledge in:
    • Negotiation and conflict management.
    • Crisis management.
    • Community resources including generic, local, state, and federal programs.
    • Determining the most integrated setting appropriate to meet the person’s needs.
    • Coordinating and facilitating planning meetings.
    • Assisting people in gaining access to services and supports.
    • Coordinating services.
    • Monitoring the provision of services to people.
    • In compliance with governing services for people with developmental disabilities.
  • Not been determined to be:
    • Simultaneously employed by another DHMH licensed agency.
    • On the Medicaid exclusion list.
    • Convicted of a crime of violence in violation of Criminal Law Article 14-101, annotated code of MD.
    • In violation of Health-General Article 7-1102 annotated code of MD.
    • A person with a criminal history which would indicate behavior potentially harmful to people receiving services documented either through criminal history records check or a criminal background check.
  • Demonstrate knowledge of Universal Precautions/OSHA and demonstrate effectiveness as a multidisciplinary team member.
  • Ability to learn and use computer systems to manage an individual's information.
  • Ability to demonstrate knowledge and skills necessary to provide appropriate services to individuals served.
  • Must demonstrate knowledge of the principles of growth and development and developmental disability.
  • Demonstrate highly effective interpersonal, written, and verbal communication skills.
  • The RCCS must have on record all required employment documents. Additionally, he/she must have all other documents listed below.
    • Application
    • Current CPR
    • Current driver’s license, good driving record, and reliable transportation.
    • Original Social Security card.
    • Work permit or Residency card.
    • Proof of Hepatitis-B series.
    • Physical examination/medical statement within the past 12 months.
    • TB test results or chest x-ray within the past 12 months.
    • Criminal background clearance
    • 3 professional letters of reference (employment is contingent upon receipt of satisfactory references).


RESPONSIBILITIES
  • Visit each client assigned to their CCS team at least semi-annually* to ascertain customer satisfaction with assigned CCS.
  • Ensure that CCSs are meeting client needs and following DDA requirements.
  • Guide each CCS as needed.
  • Provide weekly reports to the Assistant Director on each CCSs progress and training needs.
  • Update assigned QE sheets in the Shared Drive weekly.
  • Review and monitor CCSs documentation.
  • Review and monitor CCS services, client needs, and PCP updates.
  • Share resources with CCSs.
  • Attend DDA regional trainings and meetings as assigned.
  • Be available to work outside of regular work hours to meet client needs.


DUTIES:
  • QA visits - 40%
  • CCS support - 40%
  • Attend assigned trainings and meetings - 10%
  • Reports and QE sheets - 10%


WORK CONDITIONS:
  • Work involves exposure to uncomfortable and unpleasant surroundings.
  • As an “essential” employee, you may be called upon to assist with natural disasters and or public health emergencies.
  • Work involves exposure to hazardous conditions which may result in injury.
  • Working with terminally ill patients.
  • Home visits may occur in unsafe neighborhoods and/or unhygienic environments.
  • Work requires use of protective equipment such as goggles, gloves, maskes, etc, per each client’s care plan.
