Optimus Health Care is a Federally Qualified Health Care Center with sites operating in Bridgeport, Stamford, and Stratford, CT. We are the largest provider of primary health services in Fairfield County, proudly serving all patients regardless of their income, race, or insurance status.
We are presently recruiting for a Recovery Coach to provide peer support services for individuals and their families to effectively address the myriad of issues which those with substance abuse and mental health disorders require to support the recovery process.
This position is 100 % grant-funded. The successful candidate will be bilingual in English and Spanish.
Responsibilities:
• Provides recovery support through self-directed, strength-based approach to care. Utilizing principles based on the Recovery Coach Ten Components of Recovery.
• Promotes recovery by guiding Project SOAR participants through the recovery process and serving as a consistent source of support (i.e., accompanying them to 12-Step Program meetings).
• Navigates participants through or around transportation, public benefits, childcare and other barriers to successful participation in MAT services. Assisting in linkage to recovery support activities by removing personal and environmental obstacles.
• Maintains communications with community providers and builds a network of recovery support services to support program participant’s attendance in organized, social sober activities.
• Recognizes the elements of client crisis, implements an immediate course of action (in consultation with supervisor) appropriate to the crisis and utilizes negative events to enhance overall treatment outcomes.
• Prepares agenda for supervision and utilizes supervision as needed to meet professional demands of job.
• Educates individuals & families by providing information on recovery supports within the community.
• Provides information and assertive linkages to services and resources with an emphasis on self help groups, CCAR, recovery houses, sober events and programs, employment and job services.
• Identifies the needs of clients that cannot be met by the counselor/organization, explains rationale for the referral to the client, and assists the client to utilize the support systems and community resources. Meets all documentation requirements of position and program.
• Assumes all other appropriate duties as deemed necessary by supervisor.
Requirements:
• Certification as a Recovery Coach required.
• Lived experience in stable recovery of substance use disorder and/or lived experience of HIV, incarceration, and/or homelessness.
• Established expertise and practices related to professional boundaries and ethics.
• Ability to establish and maintain cooperative professional relationships.
• Good communication skills.
• Ability to project a professional image.
• Valid Connecticut driver's license is required.
