Overview
The Family Practice Nurse Practitioner (FNP) or Physician Assistant (PA) works as part of a dynamic team within an established mental health clinic. This position provides comprehensive primary care services. The FNP/PA diagnoses, treats and helps prevent acute and chronic illness. Provides physical examinations, orders and interprets lab tests, and provides acute and chronic care services to clinic patients, in accordance with Federal, State and clinic statutes and protocols. Reviews, evaluates and collaborates with psychiatric and behavioral health providers regarding treatment plans, client progress and outcomes. Will be a part of a 5 Star Patient Centered Primary Care Home (PCPCH). X-Waiver a plus for this position.
Responsibilities
Assesses patient health status and determines appropriate diagnostic and therapeutic procedures.
Monitors therapies and medications. Performs office procedures within scope of licensure and experience.
Develops and maintains therapeutic client relationships with close adherence to client-directed services.
Makes reasonable and sound medical assessments and treatment plans.
Prescribes medication and performs other therapeutic measures for the treatment of patient.
Thoroughly documents diagnosis, treatment and all aspects of such illness(es) in accordance with applicable OARs and industry standards. Documentation is done the same day the service is completed.
Orders appropriate laboratory and other special diagnostic tests and evaluates data obtained.
Provides patients and family members with counseling and education regarding health maintenance, disease prevention, diagnosis, treatment, and need for follow up as appropriate during each patient visit.
Skilled in creating patient centered, mutually collaborated plans of care. Motivational interviewing experience a plus.
Verifies and documents that the patient understands diagnosis, treatment and follow up recommendations.
Identifies special needs of clients and works with staff to address those needs. Experience working with vulnerable populations a plus.
Assesses patient needs for specialty referrals, diagnostic and out-patient care, and makes appropriate referrals for service.
Confers on the diagnosis and direction of treatment with client, significant others, agency clinicians, and with client’s other treatment providers.
Qualifications
Be eligible for Oregon license as Physician Assistant (PA) or Nurse Practitioner (NP).
Have Federal and State DEA certification (X-Waiver Preferred but not required)
2 year(s): Two years of relevant work experience preferred. Family Practice and team based care experience preferred.
Maintain an active, unrestricted Oregon professional license with prescriptive privileges and current DEA registration.
Maintain professional liability insurance coverage.
Enroll as Medicare provider for Options through CMS and participate in other third party payment programs as required.
Possess business experience sufficient to meet the demands of the position, including proficiency with a variety of software applications.
Must pass state-required background and DMV checks; possess a valid OR driver's license and a satisfactory driving record. Candidate must be able to work independently and flexibly, under general supervision.
