Primary Responsibilities:
- Complete a full history and physical assessment for patients in their homes
- Submit documentation at the completion of the assessment
- Travel to patients' homes or nursing homes
Since this position involves in-home visits, the ability to work autonomously and travel extensively will be required. You should also be comfortable and adept at completing extensive EMR documentation and management within multiple systems.

Required Qualifications:
- Active, unrestricted license to practice nursing in the state of assignment
- Ideal candidates will reside in the Tulsa county
- Nurse Practitioner certification from either the ANCC or AANP
- 1 years of experience as a Nurse Practitioner
- Valid driver's license and required liability insurance for personal vehicle or access to reliable transportation
- Must be able to effectively communicate with elderly and chronically ill patients (including patients with illnesses like Alzheimer's, dementia and mental health conditions) and their families
- Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. UnitedHealth Group will adhere to all federal, state and local regulations as well as all client requirements and will obtain necessary proof of vaccination, and boosters when applicable, prior to employment to ensure compliance. Candidates must be able to perform all essential job functions with or without reasonable accommodation
Preferred Qualifications:
- Understanding of dual populations who are enrolled in both Medicare and Medicaid is beneficial
- Geriatric or adult medicine specialty experience
- Bi-lingual or multi-lingual
