Position Summary:
The Case Manager is responsible for supporting the clinical team/therapists by performing a variety of duties and projects related to the clinical case management of Our Hope clients.
Work as a team to provide the best possible quality clinical care staying within the scope of the profession.
Essential Responsibilities:
- Greet all visitors to Our Hope, providing direction as needed. Provide a welcoming environment for all clients and guests at all times.
- Assists with client admission and discharge.
- Work with the clinical therapists and acts as a liaison between community resources, family, and other various contacts.
- Coordinates appointments for clients and transports as necessary.
- Serve as a client advocate.
- Responsible to document and report all necessary information in the clinical chart.
- Coordinates the client appointments
- Assists in the preparation of quality improvement activities and required reports.
- Provides input into the treatment planning process.
- Assists in the processing of medication scripts and pick-up as needed.
- Maintain compliance with state and federal employment regulations.
- Completes Basic Needs Assessment for each resident
- Complete administrative functions as required.
- Continuously drive and support a healthy and safe culture.
- Maintain competency in policy rules and regulations.
- Provide educational, didactic group sessions for clients, when needed.
- Utilize the Insync EMR (Electronic Medical Record) for client record keeping.
- Practice and uphold the Our Hope Standards of Excellence and Ethical Codes of Conduct.
Education & Experience:
- Bachelor's degree specializing in Social Work or related field or equivalent work experience.
- Experience in chemical dependency and mental health.
- Valid driver’s license.
- Current certification in First Aid and CPR.
Other Knowledge, Skills & Abilities:
- Knowledge of gender-specific treatment interventions.
- Demonstrated ability with word processing software and office equipment.
- Organizational and time management skills.
- Good communications skills, decision-making ability, and self-motivation.
- Ability to work with a team milieu approach to treatment of women.
- Knowledge of community resources, 12 Step programs, evidence-based practices, and counseling theories.
