This Registered Nurse Opportunity
The Registered Nurse (RN) provides day-to-day triage needs for the primary care clinic. This position works in coordination with multiple primary care providers, behavioral health providers, and care team members. Through skilled assessment, education, collaboration, and coordination of healthcare and community resources, this position helps patients achieve optimum functional health status and improve quality of life. This position also provides triage in-person or by phone as needed.
This is a full-time, exempt, union-represented position with a starting annual pay of , depending on skills and experience. Essential Duties
Work in collaboration with multidisciplinary providers and clinic team members to provide innovative, patient centered, evidence-based care.
Under provider supervision, works with patients to deliver a care plan including follow- up, labs, care management, and care coordination.
See patients for same-day acute care and follow up of ongoing problems (eg. Wound care, blood pressure rechecks, hospital follow-up).
Responsible for answering patients calls in a timely manner, including providing telephone advice per protocol, and following up with clients regarding abnormal lab results.
Triages patients in person and on the phone.
Provide clinical guidance within scope of license and acts as a resource for medical assistants as needed.
Use technology to assist with all aspects of care, including electronic medical record documentation, disease registry, documentation prompts, and standing RN specific protocols.
Completes visit documentation on time and accurately and submit encounters for billing as appropriate.
Consistently follow all standing orders and use advice protocols as a guide in assisting patients.
Actively participate in discussion and creation of evidence-based protocols.
Support the organization as needed.
Qualifications
KNOWLEDGE AND SKILLS
Knowledge
Knowledge of Patient-Centered Primary Care Home model.
Knowledge, training, skills and/or experience in providing services in a highly diverse work environment to a highly diverse and vulnerable population.
Knowledge of patient bill of rights, data privacy, record protection, HIPAA and 42CFR Part 2 regulations, and record retention policies.
Ability to properly code nurse encounters.
Microsoft Office Suite (Word, Excel, Outlook)
Proficient in Electronic Health Records systems (EPIC)
Skills
Demonstrate effective listening, motivational interviewing, harm reduction methods, and narrative skills.
Ability to work collaboratively with other providers, health professionals, and support staff.
Utilize resources, providers, and systems necessary to provide optimal care.

EDUCATION EXPERIENCE
Graduate of an accredited school of registered nursing
Current unencumbered State of Oregon license as a Registered Nurse
Proof of COVID-19 vaccination status or approved medical or religious exception on file is required.
WORKING CONDITIONS
This position is based in a standard medical clinic environment.
PHYSICAL REQUIREMENTS
This job requires operating phones, computers and other office equipment. Communicating is required on a regular basis. Moving inside the building to other offices and program delivery spaces as well as moving to surrounding sites is expected. Sporadically moves boxes and/or items weighing up to 20 pounds
Salary : $76,190 - $89,128
