Job Summary: Service Coordinator is responsible for assisting with the coordination and supervision of all agency services including, DDD, DCF and ISS/ Behavioral Health Services. The Service Coordinator will assist the Director of Operations/Director of Services in providing supervision to all clinical staff and interns. This includes other duties such as conducting in-service training, coordinating the Clinical Care and Record Review Committee, coordinating weekly staff and multidisciplinary treatment team meetings. Maintain leadership role, establish, implements, and evaluates goals and objectives for clinical services to meet and promote the standards of quality as well as contribute to the organization's mission and philosophy.
Qualifications/Educational Requirements:
- Bachelor’s degree in public health or related field, Master’s degree/Licensed Social Worker (LSW) preferred.
- At least two years experience in health care management preferable in - Home Care Management.

Responsibilities/ Essential Job Function: Employee must be able to perform the following essential Job Function- with or without reasonable accommodations.
- Assist with overseeing all clinical programs including DDD, DCF, Behavioral Health/Substance Abuse (OP, IOP, Partial Care), and group/individual counseling services.
- Responsible for providing direct weekly individual and/or group supervision to all interns.
- Completion of ALL New Jersey DDD/CDS required trainings to become a supervisor of the DDD Support Coordination Program.
- Responsible for providing support and managing other Support Coordinators within the agency by reviewing plans, making revisions with the appropriate input before plan approval.
- Ensures that the policies and procedures that guide and support the provision of services are implemented and that recommendations to improve performance are communicated to the company.
- Assist with Coordinating and Supervising the operations of all Agency Services including funding development, budget negotiations, Contract/RFQ/Grant preparation.
- Assist with the hiring process of all clinical and direct service workers; this would include the ability to hire and staff current client cases in the ISS and Respite Program.
- Responsible for informing staff of any changes, updates regarding service delivery
- Act as a system administrator to assign training requirements to all direct service staff and ensure that they meet all educational/training requirements and standard of care according to organization and State requirements.
- Participate in and assist with community activities such as health fairs, conventions, and community education programs
- Develop and implement marketing plans for company’s service offerings consistent with market analysis, implement specific measures for agency growth.
- Conduct Biopsychosocial/behavioral assessments, develop behavioral support plans, record documentation in NJSAMS or other EHR, and train and monitor staff for treatment plan implementation.

- Assess and complete a Person-Centered Planning Tool [PCPT] and information gathering packet with individual (client), their family, and team to gather information and identify goals, eligibility, outcomes and services needed.
- Coordinate and facilitate a team meeting to identify service needs and develop individual support plan [ISP].
- Coordinate with individuals, their families, and mentors to identify outcomes and utilize their budgets to achieve those outcomes.
- Ensure that the service plan addresses health and safety issues, including all assurances that are
required by DDD, DCF and Medicaid.
18. Assisting in identifying services and supports that will achieve that individual’s stated outcomes and can be accessed from a provider who is appropriately qualified by DDD to provide those services.
19. Drafting the service plan and making revision to it with the appropriate input from all members of the team as needed.
20. Assisting individuals and their families in assuring that the services they have identified will meet their desired outcome.
21. Connecting individuals and families with generic resources and natural supports.
22. Performing community outreach to include meeting with individuals and families and opening new cases as directed by the Director of Operations/Director of Services.
23. Entering all data into the electronic record and Coordinate with DDD staff [Waiver Assurance Coordinators] to approve the service plan, authorize the service and provide quality assurance.
24. Maintain complete and current individual records and other required reports, service reporting
and Documentation in compliance with Agency, State and Federal standards.
25. Provide quality case management and maintain phone and face to face contact with the individual, their support team and provider to ensure that the ISP and services are being delivered appropriately.
26. Participate in all meetings, in-service education, and trainings as required by DDD and the agency.
27. Participate in on-call schedule rotation
28. Perform other duties as assigned by the Director of Operations/Director of Services in keeping with general requirements of the position.
