Position Summary |
This position provides overall leadership for health care services for residents in the community, including leadership for the development and implementation of health services plans for each resident and directs the work of nurses and caregivers. |

Essential Functions |
General Nursing |
· Provide general care and treatment for residents as required within the scope of individual licensure and within the state · Provide nursing assessments for current and prospective residents as required by federal/state regulations and/or organizational policy · Function as a liaison and advocate for residents with physicians and outside providers · Provide on-call nursing support, delegating duties when unavailable |

Leadership and Training |
· Coordinate the selection and hiring of nurses and caregivers · Develop and manage the scheduling for the nurses and caregivers · Direct and monitor the work of caregivers, i.e., nursing assistants and medication aides, in the community · Conduct training as assigned and mandated by regulatory requirements |

Health Care Services Planning |
· Responsible for the accuracy of the Negotiated Services Agreement, including the Health Service Plan, and updating per state regulation and with any significant change |

Sales and Customer Service |
· Function as the primary point of contact regarding resident health care and/or services, proactively communicating with all stakeholders any change in resident condition · Meet with current and prospective families to communicate the provision of health care services in the community · Promote the scope of services provided within the community to external medical providers |

Minimum Qualifications |
· Board certified Licensed Practical Nurse/Registered Nurse · Three (3) years of nursing experience in a leadership role within long term care |

Preferred Qualifications |
· Five5) years of nursing experience in a leadership role within long term care · Previous nursing experience in memory care |
|
Physical Working Requirements |
· Ability to travel using personal vehicle, including at night · Able to do occasional lifting of up to 50 pounds · Able to work flexible schedule, including evenings and/or weekends · Team Member is subject to outside environment conditions occasionally, inside conditions frequently, with protection from weather conditions but not necessarily from temperature change |
