We have an exciting new opportunity for an experienced PACE for LIFE Program Director in Milford, Delaware.
Position Summary PACE for LIFE Program Director:
The PACE for LIFE Program Director has the overall responsibility for the planning, implementation, and operation of the Milford Wellness Village PACE Program, which includes the management of clinical, financial, and other supportive aspects of the organization to ensure safe, effective and efficient programs that support the respect and independence of its participants.
The PACE for LIFE Program Director is responsible for:
§ Administration of the PACE for LIFE Program and the provision of high-quality and comprehensive services
§ Supervision of staff and oversight of the Interdisciplinary Team in carrying out the plans of care for PACE participants 24/7/365
§ Achievement of census growth and fiscal alignment
§ Compliance with all Federal, State, and local regulations related to the operations and administration of PACE and the PACE Center
§ Maintenance of positive, productive long-term relationships with the community
Essential Duties and Responsibilities of the PACE for Life Program Director:
1. Manages all aspects of planning, coordination, and implementation of PACE services provided to participants in all settings.
2. Develops and implements systems and policies in the PACE for LIFE Program that meet all credentialing standards, and ensures quality and efficiency in all aspects of PACE for LIFE services.
3. Provides leadership to the Interdisciplinary Team (IDT). Promotes effective IDT decision-making in line with established PACE best practices. Actively participates in and facilitates team meetings, communicates relevant issues to the team, and ensures all staff is informed and up-to-date. Provides education, communication and support for staff, participants and families, as well as serves as a resource to all disciplines.
4. Facilitates and oversees the development of the participant treatment plan, facilitates and supervises the implementation of care plan interventions, and ensures the management and monitoring of participants’ progress and decline.
5. Ensures systems to maintain active communication with participants and families regarding satisfaction with services, specific concerns and suggestions.
6. Actively monitors data, quality indicators, and clinical outcomes to identify areas of improvement and ensures effective Quality Improvement processes. Oversees the QI Coordinator and actively participates in the QI Program.
7. Ensures effective human resource systems including orientation of new staff, ongoing training and education, performance management, and competency evaluations. Ensures adequate staffing according to budgetary requirements.
8. Oversees fulfillment of administrative and regulatory requirements, such as data reporting to State and CMS agencies, report submissions, audit preparation, and overall regulatory compliances.
9. Actively participates in marketing and community relations. Ensures targeted census goals are consistently met. Continuously develops referral sources and increases program enrollment.
10. Represents the program to State and Federal agencies. Participates in local, State and national organizations regarding PACE.
11. Ensures the availability of a comprehensive, high-quality PACE contracted provider network.
12. Coordinates with contract services to ensure understanding of the PACE model and cooperation with the care management process.
13. Ensures the financial health of the program, including assisting in preparing an annual operating budget, allocating the resources to carry out operational and programmatic activities, reviewing and interpreting monthly financial statements, developing actions to correct areas of concern, and developing strategies for the further growth and development of PACE.
14. Establishes systems to ensure accurate revenue management, including risk adjustment submission, enrollment processes, and accounts receivable processes.
15. Effectively tracks and manages expenses related to utilization, service authorizations, and Center operations.
16. Maintains open communication and reports to the Board of Directors regularly on operational, financial, and quality issues.
Qualifications for the PACE for LIFE Program Director:
1. Education:
a. Master degree in a healthcare-related field preferred
b. Degree in a clinical discipline (RN, PT, OT, RD or MSW
2. Credentials/Licensure Required:
a. Professional licensure in the State of Delaware preferred
b. Valid driver’s license required
3. Experience:
a. PACE/LIFE operational experience strongly preferred
b. Minimum of 5-10 years’ experience in healthcare and/or long-term managed care, preferably for an at-risk population and in a capitated setting
c. Minimum of 3 years’ management experience
d. Expertise in the financial aspects of health plan management, P&L management, and risk adjustment preferred
4. Special Training:
a. Knowledge of managed care, healthcare delivery systems, PACE regulations, quality improvement, and the medical and non-medical needs of the elderly
b. Must also possess good organizational skills, leadership skills, be able to mediate difficult and unusual interpersonal conflicts and work within the team setting
5. Age of Patients Rendered Care:
a. Adult and geriatric patients
